Surgical Outcomes <strong>of</strong> Pediatric Endonasal Dacryocystorhinostomy<br />

Purpose: To evaluate the surgical outcomes <strong>of</strong> endonasal dacryocystorhinotomy<br />

(DCR) for pediatric patients with nasolacrimal duct obstruction.<br />

Methods: Six eyes <strong>of</strong> 5 patients (4 girls, 1 boy; mean age <strong>of</strong> 5.8 +/- 2.2 years)<br />

underwent endonasal DCR under general anesthesia. Diagnosis was chronic<br />

dacryocystitis for 4 eyes, acute dacryocystitis for 1 eye and iatrogenic<br />

obstruction <strong>of</strong> canaliculus for 1 eye.<br />

Results: Although there was a difficulty <strong>of</strong> the surgery due to small nasal<br />

cavities, 5 in 6 eyes had successful results and no recurrence over 3 years.<br />

There was no impact for the facial growth in all patients.<br />

Conclusion: Endonasal DCR is a good option for pediatric patients with<br />

nasolacrimal duct obstruction. To confirm if bone resection gives an impact on<br />

the facial growth in children, longer follow-up should be required.<br />

Fishtail skin resection with modified Hotz procedure for Asian<br />

patients with congenital entropion accompanied by epiblepharon<br />

Introduction: Epiblepharon is one <strong>of</strong> the structural causes <strong>of</strong> congenital<br />

entropion in Asian people, because it causes traction <strong>of</strong> the lower eyelid to the<br />

upper eyelid. In this study, we developed a method <strong>of</strong> more easily designing<br />

the flap, then performed surgery in several cases. Cases: During 2010, one<br />

surgeon used this method to treat 12 eyes in 6 patients with lower lid entropion<br />

accompanied by epiblepharon (including 3 boys and 3 girls, average age is 6.3<br />

years old.) Surgical method; Firstly, the skin design were drawn with a markerpen.<br />

At the horizontal edge <strong>of</strong> the subciliary orbicularis <strong>of</strong> lower eyelid, a 2mm<br />

wide fusiform-shaped skin resection was designed from the inner canthus to<br />

the outer half <strong>of</strong> lid. Then, on the epiblepharon, a small triangle shape ‘fishtail›<br />

was added. After performing a Hotz procedure, the triangle skin flap was<br />

sutured to the triangle edge.<br />

Result: The triangle skin flap diminished the tension <strong>of</strong> the orbicularis. After a<br />

minimum <strong>of</strong> 6 months, the shape <strong>of</strong> medial canthus was opened widely, and<br />

the direction <strong>of</strong> cilia was stable.<br />

Conclusion: This blepharoplasty approach is easy to design and facilitates<br />

performance <strong>of</strong> the Hotz procedure, allowing effective correction <strong>of</strong> congenital<br />

entropion.<br />

Exophthalmometry Values in Upright and Recumbent Positions in<br />

Bandar-Abbas Population<br />

Purpose: To determine normal values <strong>of</strong> Hertel exophthalmometry in upright<br />

and recumbent position in resident population <strong>of</strong> south <strong>of</strong> Iran in Bandar-Abbas.<br />

Methods: In a population-based epidemiological study, ocular protrusion was<br />

measured in 459 randomly selected normal residents <strong>of</strong> Bandar-Abbas. Hertel<br />

exophthalmometry was performed in all by a single ophthalmologist in both<br />

upright and recumbent positions.<br />

Results: 459 normal subjects were examined. The age range was 19 month-85<br />

years (19.41+/- 14.88 year). Two hundred and thirty two (50.5%) <strong>of</strong> subjects<br />

were male. Considering both eyes, mean absolute exophthalmometric values<br />

were significantly different between upright and recumbent positions, 12.22<br />

mm (+/-1.92) in upright position and 12.01 mm (+/- 1.88) in recumbent position<br />
