Circadian and Orthostatic Influences on Cystoid Macular Edema<br />

Morphology<br />

Purpose: To identify the influence <strong>of</strong> orthostatic and diurnal changes on the<br />

morphology <strong>of</strong> cystoid macular edema (CME).<br />

Methods: Sixteen patients (18 eyes) diagnosed with CME were examined<br />

using the Heidelberg Engineering SPECTRALIS HD-OCT in 4-hour-intervals<br />

between 8am and 8pm. Examination <strong>of</strong> the exact same retinal area was<br />

ensured using Heidelberg›s proprietary follow-up function. Additionally, at 4<br />

pm an orthostatic test was performed and monitored using the Zeiss Cirrus<br />

HD-OCT.<br />

Results: Changing from a reclined into an upright position led to an immediate,<br />

significant decrease central retinal thickness (CRT) from 496±37 µm to 412±43<br />

µm, i.e. 20% within the first 20 seconds after the position change. The diurnal<br />

pattern showed a small but statistically significant decrease <strong>of</strong> CRT. The total<br />

variation was 3.5% with the highest CRT in the morning and the lowest in the<br />

evening.<br />

Conclusions: Intraretinal fluid volume in CME appears to be linked to orthostatic<br />

changes following position changes <strong>of</strong> the patient. CRT decreases significantly<br />

within seconds after moving from a reclined to an upright position. Furthermore<br />

CRT is largest in the morning hours continuously decreasing during the day,<br />

making timing an important factor to consider when comparing retinal thickness<br />

values during follow-up.<br />

Development and Triamcinolone Induced Resolution <strong>of</strong> Cystic<br />

Macular Edema in Uveitis: Correlating Morphology with Functional<br />

Treatment Outcome<br />

Gaining insight into morphological and functional aspects <strong>of</strong> uveitis-associated<br />

cystoid-macular-edema (CME) using high-resolution optical-coherencetomography<br />

(HD-OCT). Twenty-eight eyes with uveitis-associated CME were<br />

examined before and after intravitreal-triamcinolone-injection (IVTA) for 3<br />

months. Retinal morphological changes including cyst size, distribution and<br />

development, layer-integrity and subretinal-detachment (SRD) were evaluated<br />

using HD-OCT. Findings were correlated to functional aspects, such as bestcorrected-visual-acuity<br />

(BCVA). Uveitis-associated CME –in contrast to CME <strong>of</strong><br />

other origin - showed a characteristic denture-like appearance: Following IVTA,<br />

first the big foveal cysts diminished, followed by the small parafoveally located<br />

incisor-like-organized INL cysts. Fifteen <strong>of</strong> 28 eyes with CME showed SRD<br />

before treatment initiation which resolution lagged behind cyst disappearance.<br />

Within a CME-relapse, INL cysts first developed, followed by typical big<br />

centrally-located cysts, whereas no subject showed new- or reappearance <strong>of</strong><br />

SRD within the observation period. Presence <strong>of</strong> initial SRD was a predictor for<br />

a relapse free clinical course, whereas 75% <strong>of</strong> subjects without SRD showed<br />

a cyst relapse. Uveitis-associated CME presents characteristic morphologic<br />

alteration discernible from CME <strong>of</strong> other origin. Different morphologic patterns<br />

in uveitis-associated CME may represent different stages in the progression <strong>of</strong><br />

uveitis-associated CME and initial morphologic appearance can be linked to<br />

the clinical prognosis after treatment.<br />

The Utility <strong>of</strong> the Transversal OCT Scans in Central Serous<br />

Chorioretinopathy<br />

Purpose: In central serous chorioretinopathy (CSC), we evaluated the optical<br />

coherence tomography (OCT) images <strong>of</strong> the retinal pigment epithelium (RPE)<br />

obtained by scanning in a transversal direction (parallel to the retinal surface),<br />

and compared them, firstly, with the morphological findings in the unaffected<br />

eyes and secondly, with the leakage points on fluorescein angiography (FA).<br />

Methods: In the prospective study 42 eyes <strong>of</strong> 21 patients with unilateral acute<br />

CSC were examined with OCT Cirrus-HD (Macular Cube-Advanced<br />

Visualization-RPE Slice). CSC was confirmed in 21 eyes (Group A) and the<br />

contralateral 21 healthy eyes formed the Group B. FA was obtained in all<br />

patients. Central macular thickness and the major demographics were also<br />

recorded.<br />

Results: Transverse OCT images showed irregular lesions in RPE: Group A:<br />

100%; Group B: 9, 5% (2/21). The analyzed results agreed in 85, 7% (18/21)<br />

with the fluorescein dye leakage location on FA.<br />

Conclusions: In eyes with acute CSC, in contrast with conventional longitudinal<br />

OCT scans, the extent <strong>of</strong> the disease could be visualized more accurately by<br />

providing transversal slice at RPE. A good correlation between the transversal<br />

OCT findings and the FA images was demonstrated by typical hyporeflective<br />

morphologic changes at the point <strong>of</strong> dye leakage.<br />

Optimisation <strong>of</strong> RIALAB, an Automated Drusen-Quantifying<br />

S<strong>of</strong>tware, in Analysing Drusen Distribution in Age-Related Macular<br />

Purpose: Traditionally drusen in age-related macular degeneration (AMD) is<br />

difficult to quantify reliably. In this pilot study, we demonstrate application <strong>of</strong><br />

an automated drusen segmentation algorithm in delineating and quantifying<br />

drusen distribution from retinal photographs. To improve throughout to study<br />

large numbers <strong>of</strong> images, a cross-validation study was performed to optimize<br />

algorithm parameters.<br />

Methods: 123 images with drusen were selected from two epidemiological<br />

studies and run through various combinations <strong>of</strong> RIALAB algorithm parameters<br />

based on possible drusen phenotype configurations, such as distribution,<br />

hardness, and presence <strong>of</strong> pebble. The resulting output <strong>of</strong> delineated drusen<br />

was cross-checked by an observer to determine the most appropriate setting.<br />

Results: 99 images (80.5%) had the parameter Few Outer as the best setting<br />
