Outcomes After Crystalens HD Accommodative Intraocular Lens<br />

Implantation<br />

Purpose: To report the outcomes after Crystalens HD accommodative<br />

intraocular lens implantation<br />

Methods: 22 patients (44 eyes) with mean age 62, 09± 4, 49 years (range from<br />

54 to 71 years) underwent cataract surgery and bilateral Crystalens HD lens<br />

implantation.<br />

Results: Mean follow-up was 16, 62 ± 5, 58 months (range from 4 to 23 ).<br />

Uncorrected Distance Visual Acuity (UDVA) improved from 0,4 ±0,27 (range<br />

from 1 to 0) preoperatively to 0,14±0,13 (range from 0,46 to 0 ) and Distance<br />

Corrected Visual Acuity (CDVA) improved from 0,11 ± 0,15 (range from<br />

0,54 to 0 ) preoperatively to 0,03 ±0,07 (range from 0,26 to 0) (LogMar) at<br />

the last follow up. 82% <strong>of</strong> our patients had J2 or better <strong>of</strong> near vision, at<br />

the last follow up, measured with Birkhauser reading charts at 35cm under<br />

photopic conditions. 73% <strong>of</strong> our patients had J2 or better <strong>of</strong> intermediate vision,<br />

measured with Birkhauser reading charts at 60cm under photopic conditions<br />

No complications occurred intra or postoperatively.<br />

Conclusion: Crystalens HD, can restore some degree <strong>of</strong> accommodation<br />

providing satisfactory visual outcomes for distant, intermediate and near vision.<br />

Experience with a New ‹Chopper› Phaco Tip for Hard Cataracts<br />

Purpose: A new blade-like tip has been developed to allow ultrasonic chopping<br />

<strong>of</strong> a cataract. This Paper describes early experience with this tip in hard<br />

cataract cases.<br />

Methods: A series <strong>of</strong> hard cataracts was operated using the Alcon Infiniti<br />

system using OZil torsional phaco and this new tip. The tip is used to cut a<br />

narrow groove deep into the body <strong>of</strong> the nucleus. Different methods <strong>of</strong> splitting<br />

the nucleus after grooving/chopping were explored.<br />

Results/Conclusions: This new tip can effectively create a narrow groove in<br />

dense cataracts which can then be split to achieve nucleus disassembly safely.<br />

Experimental Study <strong>of</strong> Wavefront Aberrations <strong>of</strong> Aspherical<br />

Intraocular Lens<br />

Objective: To assess the effects <strong>of</strong> aspherical IOLs, including effects on visual<br />

acuity, spherical aberration, high-order aberrations (HOA),coma and trefoil.<br />

Using ZEMAX optical design program to analysis the optical quality <strong>of</strong> the IOLs<br />

in the Hwey-Lan Liou model eye.<br />

Methods: Using ZEMAX optical design program, (1) analogue monochromatic<br />

aberration <strong>of</strong> the IOLs at the length <strong>of</strong> 550nm, pupil diameters were 3mm~6mm;<br />

(2) analogue chromatic aberration <strong>of</strong> the IOLs at photopic and scotopic<br />

conditions, pupil diameters were 3mm and 4.5mm. (3) The longitudinal<br />

chromatical aberration (LCA) was estimated directly (from the defocus term in<br />

the polynominal expansion <strong>of</strong> the wavefront aberration).<br />

Results:<br />

1. With the increasement <strong>of</strong> the aperture sizes, the modulation transfer function<br />

(MTF) <strong>of</strong> the aspherical IOLs decreased, both in the monochromatic and<br />

chromatic aberration<br />

2. In the aperture <strong>of</strong> 3mm~6mm, the MTF <strong>of</strong> the aspherical IOLs were superior<br />

than that <strong>of</strong> the spherical one.<br />

3. No matter in the photopic or scotopic condition, the MTF is higher in IOL<br />

Hoya£¨Abbe=43£© than IOL SA60AT£¨Abbe=37£©<br />

Conclusion:<br />

1. Aspherical IOLs can increase the visual quality<br />

2. With the increasement <strong>of</strong> the aperture sizes, the optical performance <strong>of</strong> the<br />

aspherical IOL decreased<br />

3. The lower the chromatic dispersion <strong>of</strong> the IOL, the higher the MTF<br />

Cataract Surgery with the Modified CTR in Patients with Traumatic<br />

Zonular Dialysis and Mydriasis<br />

Purpose: To investigate the safety, visual functions, pupil appearance and final<br />

IOL position after phacoemulsification combined with Malyugin-Cionni CTR<br />

scleral suturing, in-the bag IOL fixation and iridoplasty.<br />

Methods: 6 patients (mean age 49.14+/-11.7 years) with traumatic cataract,<br />

zonulolysis and fixed dilated pupil after blunt trauma dated from 5 months to 16<br />

years. The extent <strong>of</strong> the zonular defect varied from 60 to 210 degrees (mean<br />

89 degrees) according to UBM.<br />

Results: Preoperatively BCVA varied from counting fingers to 20/70 and<br />

improved to the mean 20/30 after the surgery. No major complications were<br />

observed during the surgery. One patient required glaucoma surgery 3 months<br />

after the primary operation. The follow-up was 7.8±2.2 months. Capsular<br />

bag and IOL position were verified with ultrasonic biomicroscopy. In all <strong>of</strong> the<br />

patients IOL and the capsular bag were positioned centrally. Mean shift in<br />

the direction <strong>of</strong> the traction suture was 0.25+/-0.22 mm, lens tilt 5.39+/-4.05<br />

degrees. IOP was normal, pupil usually having a polygonal shape with 3.5mm<br />

to 4.0 mm diameter.<br />

Conclusions: Combining the CTR scleral suturing with clear corneal phaco and<br />

iridoplasty provided favorable functional and cosmetic results.<br />
