Collagen Crosslinking in Early Keratoconus with Rib<strong>of</strong>lavin in a<br />

Femtosecond Laser-Created Pocket.Two years <strong>of</strong> Follow Up<br />

Purpose: To evaluate the safety and efficacy <strong>of</strong> a novel femtosecond laserassisted<br />

technique for intrastromal administration <strong>of</strong> rib<strong>of</strong>lavin and ultraviolet A<br />

(UVA) light in collagen crosslinking (CXL) for keratoconus.<br />

Setting/Venue: Protypo Ophthalmiatrio Thessaloniki.<br />

Methods: This method uses a femtosecond laser-assisted technique for direct<br />

intrastromal administration <strong>of</strong> rib<strong>of</strong>lavin in collagen crosslinking (CXL) for<br />

keratoconus. Using femtosecond laser, a 5,5mm diameter circular intrastromal<br />

pocket is created. There is a clear optical zone <strong>of</strong> 3mm .Thus a circular pocket<br />

at 200? depth and 2,5mm width is created. Then 0.1-mL doses <strong>of</strong> 0.1%<br />

rib<strong>of</strong>lavin solution infused into the pocket twice. UVA irradiation will follow with<br />

7 mW/cm (2) UVA light 370 nm wavelength. At the same time rib<strong>of</strong>lavin is<br />

instilled at the epithelium every ten minutes for half an hour.<br />

Results: To be announced. Parameters to be evaluated are BCVA, Kmax,<br />

Corneal pachymetry, endothelial cell count, Refractive correction, Asymmetry<br />

index.<br />

Conclusions: This method avoids epithelial debridement with fast visual<br />

rehabilitation and minimal discomfort.<br />

Close Chamber Penetrating Keratoplasty Technique, a New<br />

Innovation in Keratoplasty Surgery<br />

Purpose: Penetrating keratoplasty (PK) is an open chamber technique which is<br />

a challenging procedure for any corneal surgeon, until put one or two stay<br />

sutures which is not under surgeon’s control . This method is adopted to<br />

minimize complications related to conventional PK.<br />

Method: Group A included 37 cases as conventional PK where the recipient<br />

cornea is cut completely and then the donor button was put and stitched<br />

usually 12 or 16 interrupted sutures taken with 10-O nylon. Group B included<br />

23 cases, the recipient cornea was cut half way from 12 to 6 O’ clock then the<br />

donor button stitched to the same site with interrupted sutures. Thus recipient<br />

corneal removal and donor graft suturing was done simultaneously. This is<br />

close a chamber PK. All these are demonstrated in videos.<br />

Result: In the Conventional PK case (group A) there was significantly more<br />
