Efficency <strong>of</strong> Supratarsal Triamcinolone Injection in the Management<br />

<strong>of</strong> Refractory Vernal Catarrh<br />

Objective/Purpose: To analyse 24 months results <strong>of</strong> the new rotatory<br />

asymmetric bifocal IOL not depended on pupil size.<br />

Methods: Oculentis Mplus IOLs were implanted in 25 patients after standard<br />

phaco cataract surgery, 49 IOLs in total. UCVA at far, middle and near distance<br />

(ETDRS charts for 6m, 100cm and 40cm) and contrast sensitivity (Pelli Robson<br />

Contrast Sensitivity Test) were measured. Defocus curve was measured in 37<br />

eyes.<br />

Results: UCVA at far distance increased after 7 days from 0.34 to 1.08. UCVA<br />

at near distance was 0.80. PRCST increased from 1.35 to 1.85. Halo and glare<br />

light phenomenons observed 8 patients in the lower part <strong>of</strong> the light sources.<br />

One patient has a disturbing effect during night driving. Three patients use<br />

glasses for far distance (one myopic and 2 hyperopic shifts), 5 patients use<br />

glasses for near distance but only for special conditions. In two eyes the<br />

secondary cataract (proliferative form) was successfully removed one year<br />

after surgery. All patients were satisfied with the new quality <strong>of</strong> vision.<br />

Conclusion: New MF IOL has acceptable long term results without loss <strong>of</strong><br />

contrast sensitivity and disturbing light phenomenon.<br />

Ocular Infestation by Dir<strong>of</strong>ilaria. Repens<br />

Objective: This is a case report describing a case <strong>of</strong> Dir<strong>of</strong>ilaria which presented<br />

to us as conjunctivitis, treated elsewhere for the same. Reports from India are<br />

limited. Dir<strong>of</strong>ilarial ocular manifestations have been reported as early as 1885.<br />

Case Report: A 55 year old lady from South India, reported to our Hospital with<br />

foreign body sensation, pain and redness in the left eye since 3days.<br />

On examination a diffuse conjunctival congestion was noted. A slit lamp<br />

examination revealed a live motile worm in the temporal subconjunctival space.<br />

Rest <strong>of</strong> the ocular examination was normal. Systemic examination revealed<br />

no other nodules, no lymphadenopathy. Routine blood investigations were<br />

normal, including eosinophil count. Blood smear was negative for micr<strong>of</strong>ilaria.<br />

The live worm was removed under local anesthesia. It measured 12cm in<br />

length, 0.45cm in diameter. Specimen was identified as Dir<strong>of</strong>ilaria repens.<br />

Conclusion: This case brings to our notice the possibility <strong>of</strong> the endimicity <strong>of</strong><br />

dir<strong>of</strong>ilaria as evident from previous reports from the same area. Dir<strong>of</strong>ilaria<br />

because <strong>of</strong> its masquerading abilities, mimics a myriad <strong>of</strong> ocular diseases. The<br />

prompt diagnosis and treatment resulted in a symptom free patient.<br />

Corneal Biomechanical Properties Distribution in Myopic Population<br />

Purpose: To evaluate distribution <strong>of</strong> corneal biomechanical measurements<br />

(Corneal Hysteresis = CH, Corneal Resistance Factor = CRF) in myopia and<br />

myopic-astigmatism population.<br />

Methods: 180 eyes with myopia and myopic-astigmatism candidated for laser<br />

refractive surgery included in this study. Complete examination <strong>of</strong> anterior<br />

and posterior segments, manifest refraction spherical equivalent (MRSE)<br />

, OrbscanII and Zywave were performed preoperatively. Ocular Response<br />

Analyzer was used to measure CH and CRF. Distribution <strong>of</strong> all corneal<br />

biomechanical properties and correlation between these parameters and<br />

MRSE, age and sex were determined.<br />

Results: Mean age was 28 7 years. Mean MRSE was -4.21D. Mean CH and<br />

CRF, was 10 mmHg and 10.16 mmHg , respectively.28.4% <strong>of</strong> all myopic<br />

population had CH about 10mmHg, and CRF in 25.9% <strong>of</strong> myopic population<br />

was 10mmHg.There was no statistically significant correlation between MRSE<br />

& CH ( Rs= 0.001 , P= 0.71 ) and MRSE & CRF ( Rs= 0.01 , P= 0.18 ).<br />

Conclusion: Our study demonstrated the distribution <strong>of</strong> corneal biomechanical<br />

properties. In normal myopia and myopic - astigmatism population in IRAN,<br />

and confirmed that, there was no statistically significant correlation between<br />

corneal biomechanical properties and MRSE , age and sex.<br />

A Rare Case <strong>of</strong> Sebaceous Carcinoma <strong>of</strong> the Caruncle<br />

Purpose: To report a rare case <strong>of</strong> sebaceous carcinoma <strong>of</strong> the caruncle.<br />

Methods: A 50-year-old man developed a slowly enlarging mass in his left<br />

caruncle. On the basis <strong>of</strong> the clinical findings, we suspected it to be a<br />

papillomatous lesion. However malignancy could not be ruled out, so, total<br />

wide excisional biopsy with frozen section control was undertaken and the<br />

lesion was studied histopathologically.<br />

Results: Histopathological examination disclosed infiltration <strong>of</strong> the caruncular<br />

stroma by lobules <strong>of</strong> a malignant neoplasm that had prominent cytoplasmic<br />

vacuoles, typical <strong>of</strong> well differentiated sebaceous carcinoma.<br />

Conclusion: Although it is best known to arise in the meibomian glands <strong>of</strong> the<br />

tarsus, sebaceous carcinoma can very rarely arise in the caruncle and can be<br />

misinterpreted clinically and even histopathologically. This report suggests the<br />

importance <strong>of</strong> considering sebaceous carcinoma in the differential diagnosis<br />

<strong>of</strong> such benign looking lesions in this rare site. In our case we believe, early<br />

detection and radical excision <strong>of</strong> the tumour may yield a better prognosis for<br />

the patient.
