Plaque Therapy for Ocular Tumors: Middle East - Single Institute<br />

Purpose: To analyze the three years experience and results <strong>of</strong> the Plaque<br />

Therapy (PT) in Ocular Tumors (OT) at single institute (King Hussein Cancer<br />

Center-KHCC-) in a developing country (Jordan) from the Middle East region.<br />

Methods: A review <strong>of</strong> our early results <strong>of</strong> PT service at KHCC.<br />

Results: During Mar. 2008 till Nov. 2010, the team did twelve procedures for<br />

different cases <strong>of</strong> OT; 25% Retinoblastoma in children and 75% Choroidal<br />

Melanoma in adults, treated with a prescribed dose <strong>of</strong> 4400cGy and 8500cGy<br />

respectively, 75% were Jordanian and 25% were from neighboring countries.<br />

The 2-years eye preservation rate was (92%), dose to optic disk and to macula<br />

were inversely related to the distance between the tumor edge and the optic<br />

disk and the macula (P=0.012 and 0.006; respectively).<br />

Conclusions: PT was successfully implemented at KHCC and this service can<br />

be applied easily in other developing countries. This service helped our national<br />

patients and those from neighboring countries. Close integration with other<br />

advanced programs is mandatory for this implementation in order to safely and<br />

correctly transfer this technology. Our preliminary results were encouraging<br />

and were similar to other programs worldwide.<br />

No Chlamydiae or Helicobacter Pylori is Detectable in a Cohort <strong>of</strong><br />

Chinese Patients with Conjunctival MALT Lymphoma<br />

Purpose: To detect microbial pathogens in conjunctival mucosa-associated<br />

lymphoid tissue (MALT) lymphoma specimens in an attempt to find out possible<br />

associations between conjunctival MALT lymphoma and microbial infections.<br />

Methods: Using the PCR technique, freshly obtained tumor specimens from<br />

16 cases <strong>of</strong> conjunctival MALT lymphoma as confirmed by postoperative<br />

pathology were detected for DNA <strong>of</strong> Chlamydia psittaci (Cp), Chlamydia<br />

trachomatis (Ct), Chlamydia pneumoniae (Cpn) and Helicobacter pylori (Hp).<br />

Synthetic Cp, Ct, Cpn and Hp DNAs were used as positive control, and blank<br />

plasmid DNA as negative control.<br />

Results: Electrophoresis showed that no bands corresponding to the positive<br />

control were observed in the specimens, indicating that no DNA <strong>of</strong> the four<br />

microbials was detected in the specimens <strong>of</strong> the 16 cases <strong>of</strong> conjunctival MALT<br />

lymphoma.<br />

Conclusion: The PCR technique was able to detect the positive control quickly<br />

and accurately, but the results <strong>of</strong> PCR in detecting the 16 specimens were<br />

negative, indicating that there is no association between conjunctival MALT<br />

lymphoma and the four microbials in Chinese patients.<br />

Brachytherapy for Ocular Melanoma During Pregnancy<br />

Objective: To report the outcome <strong>of</strong> brachytherapy for uveal melanoma during<br />

pregnancy.<br />

Methods: A case report <strong>of</strong> two white females aged 21 and 26 years, who<br />

presented during their 15th and 22nd week <strong>of</strong> gestation with choroidal<br />

melanoma, respectively. B-scan ultrasonography showed a tumor 8.5 and 10.1<br />

mm in height and 12.3 and 14.1 mm in diameter, respectively, there was no<br />

extraocular extension. Iodine brachytherapy <strong>of</strong> 80 and 70 Gy was delivered in<br />

138 and 266 hours, respectively.<br />

Results: The fetal doze in the first case was estimated to be 1-2 mSv by the<br />

National Radiation Safety Authority. In the second case the plaque was<br />

irradiating directly away from the fetus and the fetal dose was close to nil.<br />

One patient delivered a full-term normal baby, the other twins. Both choroidal<br />

melanomas responded well and one year after treatment tumor height was 5.8<br />

mm and 4.6 mm, respectively. However, both patients later developed liver<br />

metastasis and one subsequently died <strong>of</strong> progressive metastasis. No local<br />

tumor recurrence has taken place.<br />

Conclusion: Brachytherapy <strong>of</strong> selected tumors is safe to the fetus, provided<br />

that calculation <strong>of</strong> the fetal dose is done.<br />

Prognostic Factor Analysis in Vitreoretinal Surgery for Retinal<br />

Detachment Attributable to Macular Hole in High Myopia<br />

Objective: To detect the prognostic factors associated with initial reattachment<br />

after vitreoretinal surgery for retinal detachment attributable to macular hole<br />

(MHRD) in high myopia.<br />

Methods: This study included 85 eyes <strong>of</strong> 84 patients with MHRD in high<br />

myopia (diopter more than -8.0 or axial length more than 26.0mm). We<br />

performed multivariate logistic regression analysis to assess the influence <strong>of</strong><br />

each preoperative factor on initial success.<br />

Results: Success rate <strong>of</strong> initial reattachment was 83.53%. Postoperative bestcorrected<br />

visual acuity (BCVA) <strong>of</strong> 71 eyes with initial success was significantly<br />

better than those <strong>of</strong> 14 eyes with initial failure (P
