is crucial as such preset procedures may not be found in the literature. In this<br />

presentation, author describes few <strong>of</strong> his challenging and difficult cases and<br />

different tricks and tips to handle such cases. Eyelid reconstruction depends<br />

How to Salvage an infected Orbit<br />

To salvage an infected orbit is important to prevent the life <strong>of</strong> a patient.<br />

Aggressive antimicrobial treatment and early intervention for pus formation<br />

will produce the best outcome with least complications. In this presentation<br />

46 cases <strong>of</strong> orbital infections managed at our institution will be discussed.<br />

This will include the treatment strategies for paranasal sinus mucoceles,<br />

panophthalmitis, Mucormycosis, Aspergillosis, tuberculosis and hydatid cyst.<br />

The investigation and treatment strategies will be discussed in detail with the<br />

latest updates in literature.<br />

Topical Therapies for Eyelid Tumors<br />

Purpose: To discuss topical therapies for eyelid tumors.<br />

Methods: Literature review.<br />

Results: Some eyelid tumors can be treated with topical medications. These<br />

include Propranolol for capillary hemangioma <strong>of</strong> infancy, Imiquimod for basal<br />

cell carcinoma (BCC), squamous cell carcinoma (SCC), Mitomycin C for<br />

sebaceous carcinoma, and Imiquimod for lentigo maligna. Interferon injection<br />

is very effective in the management <strong>of</strong> SCC, some cases <strong>of</strong> BCC, and Kaposi<br />

Sarcoma. Lastly, aspiration and injection <strong>of</strong> tissue glue can be useful for<br />

lymphangioma.<br />

Conclusion: Topical therapies can be beneficial for eyelid tumors.<br />

Author aims to raise awareness amongst the ophthalmologists regarding need<br />

<strong>of</strong> accurate pre-operative assessment, correct decision about surgical<br />

intervention and post-operative management. One should request CT scan and<br />

give antibiotics. Surgery would be done if intractable diplopia, enophthalmos<br />

and a fracture > 30%. Pure OBF can be operated by Ophthalmic Surgeon.<br />

Porous, teflon or X-Ray film plates can be used. A few complications like<br />

pyogenic granuloma, diplopia and migration <strong>of</strong> the implant are encountered.<br />

Jaw winking ptosis presents the challenge <strong>of</strong> abolishing jaw winking and<br />

correcting ptosis without creating asymmetry in any direction <strong>of</strong> gaze. The talk<br />

will present an evaluation <strong>of</strong> all available techniques. It will bring out the merits<br />

<strong>of</strong> bilateral levator excision with bilateral fascia lata sling surgery in the light <strong>of</strong><br />

experience in over 150 cases.<br />

Orbital Decompression Surgery in TED: Customising and Minimising<br />

Modern orbital decompression can be performed via hidden or small incisions.<br />

The orbital floor can be accessed trans-conjunctivally, the medial orbital wall<br />

via a trans-caruncular incision and the lateral wall via skin crease incision<br />

or lateral canthotomy approach. The number <strong>of</strong> and which orbital walls to<br />

be decompressed depends on the severity <strong>of</strong> proptosis, the preference<br />

<strong>of</strong> the orbital surgeon and the status <strong>of</strong> the paranasal sinuses. Orbital fat<br />

decompression can be performed alone (in some patients) or in combination<br />

with bone decompression trans-conjunctivally.<br />

Innovations in Socket Reconstruction<br />

Orbital surgery is a complex procedure needing combination <strong>of</strong> multiple<br />

procedures including implants, fornix formation and lid surgeries. In our<br />

study we discuss innovative indigenous PMMA orbital implants designed and<br />

manufactured in Pakistan for filling up empty sockets. Its unique design and<br />

shape help in cosmetic rehabilitation. In addition another innovation regarding<br />

complex fornix formation is being presented which provides a unique, effective<br />

and easy method for 360 degree fornix formation using superglue technique.<br />

The method is cheap, practical and easily learnt.<br />

Dry Eye: Is it a True Inflammatory Disease <strong>of</strong> the Ocular Surface<br />

The traditional approach to treat dry eye that focuses on tear replacement with<br />

artificial tears can be considered palliative in that it does not address the<br />

immuno-inflammatory process which underlies the disease. The aim <strong>of</strong> this<br />

presentation is to analyze data on the inflammatory process affecting the<br />

lacrimal gland-ocular surface functional unit, how to diagnose inflammation in<br />

tears and epithelia, and the role <strong>of</strong> an integrated approach to treat dry eye<br />

including new anti-inflammatory treatments.<br />
