IOL Exchange for Refractive Surprises<br />

Especially in very long and short eyes, the targeted refraction after cataract<br />

surgery cannot always be achieved. In these cases the surgeon can<br />

choose between different options to increase the patients› visual outcome.<br />

Options include corneal refractive surgery, the implantation <strong>of</strong> pseudophakic<br />

supplementary intraocular lenses and the exchange <strong>of</strong> the implanted<br />

intraocular lens. The lecture summarizes indications for this exchange and<br />

explains surgical procedures including their risks and complications.<br />

Is British Drivers License Rule for Central Scotoma Too Strict for<br />

Japanese Glaucoma Patients Who Are Actually Driving?<br />

Purpose: To know if the British driver›s license rules are too strict for the<br />

Japanese. It is unacceptable if there are 4 continuous scotomata within 20<br />

degrees from fixation by binocular Esterman. To know the relation between<br />

binocular Esterman Score (BES) and Functional Field Score (FFS) for the<br />

early glaucoma and NEI VFQ25 and Eli Peli›s driver›s questionnaires (EPDQ).<br />

Method: 61 glaucoma patients (female 14, male 47, average age 67}11 year<br />

old) with Japanese driver›s license, Esterman binocular program by Humphrey<br />

perimeter and Goldmann III4/e test were performed. BES and FFS were<br />

obtained. VFQ25 and EPDQ for low vision drivers were collected.<br />

Result: 6 patients failed the British rule. BES was 86}16, FFS 92}9.6. The<br />

others (55 patients) passed and BES was 94}9.6 and FFS 93±14. There<br />

were no significant differences. To EPDQ, the failed scored 0.3}0.6 and the<br />

passed 2.3}3.3. The passed scored higher, feeling dangerous about driving.<br />

The correlation against VFQ25 was better in BES rather than FFS in early<br />

glaucoma.<br />

Conclusion: The British rule seems too strict for the Japanese. For early<br />

glaucoma cases there was a better correlation for ES than FFS against VFQ25.<br />

Early Surgical Intervention with the Ahmed Glaucoma Valve (AGV) in<br />

Uncontrolled Closed Globe Injuries: A Retrospective Case Analysis<br />

Purpose: Analyze outcome <strong>of</strong> early intervention by AGV in post-contusion<br />

glaucoma. Inclusion criteria: persistent IOP ?35mmHg for 3weeks or nonresolving<br />

total hyphema.<br />

Methods: 21 adults presenting between Mar 2008-Mar 2011 with contusion<br />

injury. Average age: 34.5yrs. No other co-morbidity. 12 had raised IOP at<br />

presentation and 9 developed ocular hypertension within 1 week <strong>of</strong> the injury.<br />

The mean peak IOP was poorly controlled with maximal medication. Clinical<br />

features: 15 had hyphema, 7 mydriasis, 16 had angle recession, 2 irido-dialysis<br />

and 6 had retinal ischemia. Exclusion criteria: pre-existing glaucoma, uveitis or<br />

long-term steroids. Surgical intervention: Mean intervention was 28th day posttrauma.<br />

Ahmed glaucoma valve (model FP7) was implanted.<br />

Results: Mean IOP 1st post-op was 13mmHg. Cases were reviewed over 1<br />

year. Mean onset <strong>of</strong> hypertensive phase was 42 days post-surgery followed by<br />

stabilization phase (mean 26 weeks). At 1 year, 19 eyes achieved IOP control<br />

with/without drug (7 without drug). Mean IOP difference: 18.6 (pre and postoperative).<br />

Analysis by Kaplan-Meier method.<br />

Conclusion: Acute sustained IOP rise leads to trabecular injury and high risk <strong>of</strong><br />

nerve damage. This study points to a favorable outcome <strong>of</strong> cases undergoing<br />

early intervention and controlled decompression with AGV implantation.<br />

The Effect <strong>of</strong> Ahmed Glaucoma Valve Implantation Wrapped with<br />

Human Amniotie Membrane in the Treatment <strong>of</strong> Refractory Glaucoma<br />

Purpose: To investigate the clinical outcomes and Security <strong>of</strong> Ahmed glaucoma<br />

valve implantation (AGV) Wrapped with homemade placenta amniotic<br />

membrane in the treatment <strong>of</strong> refractory glaucoma.<br />

Methods: 40 patients (42 eyes) suffered from refractory glaucoma were<br />

randomly divided into two groups: AGV was in group B, homemade placenta<br />

amniotic membrane was applied in group A. Intraocular pressure (IOP),the<br />

forms <strong>of</strong> the filtering blebs and the post-operative complications were analyzed<br />

retrospectively for 1-12 months after Surgery.<br />

Results: The IOP in the group B was significantly higher than that in the group<br />

A (F=3.201, p±3 was<br />

demonstrated in 23 eyes (23.7%) <strong>of</strong> POAG and 23 eyes (37%) <strong>of</strong> PACG, who<br />

were on medical management. Correlation <strong>of</strong> IOP change between right and<br />

left eyes was significant in both POAG and PACD groups.<br />

Conclusion: Undilated IOP on baseline and follow-up are the standards for<br />

target IOP estimation. PACD eyes have significant IOP change after dilatation<br />

compared to POAG eyes.<br />

Continuous IOP Monitoring in Glaucoma Patients Treated with<br />

Tafluprost<br />

Objective: To investigate the ability <strong>of</strong> the Triggerfish® (Sensimed Inc) sensor<br />

to continuously monitor intraocular (IOP) fluctuations throughout twenty four<br />

hours, and to assess the effect <strong>of</strong> tafluprost (Saflutan®, Santen Inc) on twenty<br />

four hour IOP pr<strong>of</strong>ile.<br />

Methods: Twenty patients <strong>of</strong> glaucoma treated with once-daily tafluprost drops<br />

underwent twenty four hour monitoring with the sensor. Recordings obtained<br />

were evaluated for fluctuations.<br />
