Many experimental and clinical studies have largely shown that the long-term<br />

use <strong>of</strong> topical drugs may cause ocular discomfort, tear film instability,<br />

conjunctival inflammation, subconjunctival fibrosis, corneal surface impairment,<br />

and the potential risk <strong>of</strong> failure for further glaucoma surgery. The respective<br />

roles <strong>of</strong> the active compound and the preservative in inducing such toxic and/<br />

or proinflammatory effects are still being debated, but the most frequently used<br />

preservative, benzalkonium chloride, has consistently demonstrated its toxic<br />

effects in laboratory, experimental, and clinical studies.<br />

This nonrandomized, prospective, comparative study compares the optic<br />

nerve head (ONH) topographic measurements between a group <strong>of</strong> ocular<br />

hypertensive eyes (48 eyes)and a control normal group (54 eyes) . Our study<br />

showed significant lowering in four stereometric parameters: RV, HVC, RNFL<br />

th, RNFL csa in ocular hypertensive eyes compared to normal eyes. Imaging<br />

findings <strong>of</strong> the ONH using the HRT might contribute to early diagnosis <strong>of</strong> ocular<br />

hypertension converting to glaucoma.<br />

Objectives: The course aims to instruct attendees about how to deal with a<br />

patient with end-stage glaucoma in terms <strong>of</strong> clinical assessment <strong>of</strong> diagnostic<br />

and progression parameters and various modalities <strong>of</strong> treatment <strong>of</strong> such patient<br />

and the impact <strong>of</strong> treatment and disease progression on visual prognosis.<br />

Summary <strong>of</strong> content: The course will discuss an important topic that carries a<br />

major challenge in many regions <strong>of</strong> the world where a plenty <strong>of</strong> patients are<br />

seen for the first time with a severe form <strong>of</strong> glaucoma (end-stage). What<br />

are the criteria <strong>of</strong> defining advanced to end-stage glaucoma? What further<br />

investigations should be performed for these patients? And the evaluation<br />

<strong>of</strong> progression rates and continued visual loss. Evaluation and visibility <strong>of</strong><br />

medical treatment with its recent advances will be addressed and adherence<br />

to treatment plan and follow-up schedule. Surgical treatment <strong>of</strong> these patients<br />

will be discussed in details with various techniques to achieve a very low target<br />

pressure, surgical hazards, wipe out phenomenon, and long-term glaucoma<br />

stability versus continued progression and visual loss in this highly advanced<br />

disease.<br />
