equipment and facilities may include but are not limited to cable(s), equipment, and<br />

point of termination connections. <strong>TC</strong> SYSTEMS and its selected Bell<strong>South</strong> Certified<br />

Supplier must follow and comply with all Bell<strong>South</strong> requirements outlined in<br />

Bell<strong>South</strong>’s TR 73503, TR 73519, TR 73572, and TR 73564.<br />

5.12 Bell<strong>South</strong>’s Access to Remote Collocation Space. From time to time Bell<strong>South</strong> may<br />

require access to the Remote Collocation Space. Bell<strong>South</strong> retains the right to access<br />

the Remote Collocation Space for the purpose of making Bell<strong>South</strong> equipment and<br />

Remote Site Location modifications. Except in case of emergency, Bell<strong>South</strong> will give<br />

notice to <strong>TC</strong> SYSTEMS at least forty-eight (48) hours before access to the Remote<br />

Collocation Space is required. <strong>TC</strong> SYSTEMS may elect to be present whenever<br />

Bell<strong>South</strong> performs work in the Collocation Space. The Parties agree that <strong>TC</strong><br />

SYSTEMS will not bear any of the expense associated with this work.<br />

5.13 Access. Pursuant to Section 12, <strong>TC</strong> SYSTEMS shall have access to the Remote<br />

Collocation Space twenty-four (24) hours a day, seven (7) days a week. <strong>TC</strong><br />

SYSTEMS agrees to provide the name and social security number or date of birth or<br />

driver’s license number of each employee, supplier, or agents of <strong>TC</strong> SYSTEMS or <strong>TC</strong><br />

SYSTEMS’s Guests to be provided with access keys or cards (“Access Keys”) prior<br />

to the issuance of said Access Keys using form RF-2906-C “CLEC and CLEC<br />

Certified Supplier Access Request and Acknowledgement”. Key acknowledgement<br />

forms, “Collocation Acknowledgement Sheet” for access cards and “Key<br />

Acknowledgement Form” for keys, must be signed by <strong>TC</strong> SYSTEMS and returned to<br />

Bell<strong>South</strong> Access Management within fifteen (15) calendar days of <strong>TC</strong> SYSTEMS’s<br />

receipt. Failure to return properly acknowledged forms will result in the holding of<br />

subsequent requests until acknowledgements are current. Access Keys shall not be<br />

duplicated under any circumstances. <strong>TC</strong> SYSTEMS agrees to be responsible for all<br />

Access Keys and for the return of all said Access Keys in the possession of <strong>TC</strong><br />

SYSTEMS’s employees, suppliers, Guests, or agents after termination of the<br />

employment relationship, contractual obligation with <strong>TC</strong> SYSTEMS or upon the<br />

termination of this Attachment or the termination of occupancy of an individual<br />

Remote Collocation Space arrangement.<br />

5.13.1 Bell<strong>South</strong> will permit one accompanied site visit to <strong>TC</strong> SYSTEMS’s designated<br />

collocation arrangement location after receipt of the BFFO without charge to <strong>TC</strong><br />

SYSTEMS. <strong>TC</strong> SYSTEMS must submit to Bell<strong>South</strong> the completed Access Control<br />

Request Form for all employees or agents requiring access to the Bell<strong>South</strong> Remote<br />

Site Location a minimum of thirty (30) calendar days prior to the date <strong>TC</strong> SYSTEMS<br />

desires access to the Remote Collocation Space. In order to permit reasonable access<br />

during construction of the Remote Collocation Space, <strong>TC</strong> SYSTEMS may submit<br />

such a request at any time subsequent to Bell<strong>South</strong>’s receipt of the BFFO. In the<br />

event <strong>TC</strong> SYSTEMS desires access to the Remote Collocation Space after submitting<br />

such a request but prior to access being approved, in addition to the first accompanied<br />

free visit, Bell<strong>South</strong> shall permit <strong>TC</strong> SYSTEMS to access the Remote Collocation<br />

Space accompanied by a security escort at <strong>TC</strong> SYSTEMS’s expense. <strong>TC</strong> SYSTEMS<br />
