STI MANAGEMENT<br />

A 20-year-old male comes to the clinic<br />

<strong>for</strong> treatment.<br />

Using Etiologic Management<br />

The provider takes a history, does a<br />

physical exam, and notes a thick<br />

discharge from the penis. With a drop of<br />

the discharge, s/he makes a slide so a<br />

gram stain can be conducted<br />

immediately. The provider takes another<br />

sample of discharge to be tested later <strong>for</strong><br />

chlamydia, the results of which will be<br />

ready in one week. The patient waits <strong>for</strong><br />

two hours <strong>for</strong> the results of the gram<br />

stain, which is positive <strong>for</strong> gonorrhea.<br />

The provider gives treatment <strong>for</strong><br />

gonorrhea and asks the patient to return<br />

in one week <strong>for</strong> results of the chlamydia<br />

test. The patient is asked to bring his<br />

partner <strong>for</strong> treatment and is counseled<br />

and given condoms.<br />

Using Clinical Management<br />

The provider takes a history and does a<br />

physical exam. If s/he sees a urethral<br />

discharge, s/he may diagnose gonorrhea<br />

because the discharge is thick and<br />

yellow in color. S/he treats the patient <strong>for</strong><br />

gonorrhea and asks the patient to bring<br />

his partner(s) in <strong>for</strong> treatment. The<br />

provider counsels the patient and gives<br />

him condoms.<br />

SMALL GROUP WORK/LARGE<br />

GROUP DISCUSSION (50 MIN.)<br />

The trainer should:<br />

• Describe the following case to Px:<br />

A 20 year-old single man complains<br />

of burning on urination, discharge<br />

from his penis, and says he had a<br />

new sexual partner three days ago.<br />

On examination of the urethra, a thick,<br />

yellowish discharge can be seen.<br />

• Divide Px into 3 groups and give<br />

each group markers and a piece of<br />

the flipchart.<br />

• Ask each group to do the following:<br />

Choose one person to write on a<br />

piece of flipchart and another to<br />

present to the large group.<br />

Discuss and describe the<br />

diagnosis and treatment of the<br />

patient in the case study using<br />

etiologic management (group 1),<br />

clinical management (group 2),<br />

and syndromic management<br />

(group 3). Include time, resources,<br />

and training needed. (5 min.)<br />

Discuss and list separately the<br />

advantages and disadvantages of<br />

the STI management approach<br />

assigned to the group. (25 min.)<br />

• Reconvene the large group.<br />
