Menstrual History (female clients
only)
A complete menstrual history should
include:
• The date of menarche.
• Frequency and regularity of
menstrual cycles.
Training/Learning Methods

• Review the Reproductive History
Form (Px Handout 5.1).

Ask Px, what questions might be
added or deleted
• Date of onset of the most recent
period or bleeding episode.
• An estimate of the number of pads
used each day.
• Whether the adolescent has cramps
or pain, clotting, or symptoms of
dizziness or nausea with menses.
• Whether the adolescent has unusual
vaginal discharge or difficult urination.
Obstetric History (female clients only,
if applicable)
An obstetric history should include:
• Number of children she has.
• Number of times she's been pregnant.
• Her delivery history.
Physical History
A physical history should cover:
• Any current or past physical problems
and their onset, duration, and
progression.
• Whether the client thinks s/he is too
heavy or too thin.
• If the client has questions about how
her/his body is growing.
