TRAITEMENT ANTIBIOTIQUE<br />
DE L'ANGINE AIGUË<br />
OBJECTIFS :<br />
- Eliminer le streptocoque ß hémolytique <strong>du</strong><br />
groupe A<br />
- Raccourcir la <strong>du</strong>rée de l'évolution clinique<br />
CRITÈRES D'EFFICACITÉ :<br />
- Disparition <strong>du</strong> rhumatisme articulaire aigu<br />
- Éradication <strong>du</strong> streptocoque ß hémolytique<br />
groupe A.<br />
ANTIBIOTIQUES :<br />
- Pénicillines : antibiotiques de référence.<br />
Pour prévenir le rhumatisme articulaire aigu, le<br />
traitement doit <strong>du</strong>rer 10 jours<br />
+ Pénicilline V : 50 000 à 100 000 UI/kg/jour<br />
chez l'enfant. 3 millions d'UI/jour chez l'a<strong>du</strong>lte.<br />
Pour assurer une bonne observance, l'alternative<br />
est «Extencilline»:<br />
injection IM de 600 000 d'UI chez l'enfant - 25<br />
kg<br />
1 million 200 000 au-delà.<br />
- Macrolides : intérêt en cas d'allergie<br />
Taux de résistance : 1. 5 %<br />
Posologie : 30 à 50 mg/kg/jour en 2 à 3 prises.<br />
- Céphalosporines orales de 1ère<br />
génération, Aminopénicillines : leur efficacité<br />
est identique à la Pénicilline ; l'inconvénient est<br />
leur large spectre.<br />
