FULL· TIME HOURLY PROFESSIONAL EMPLOYEE'S LIMITED CONTRACT
Agreement between the Board of Education of the City School District of Columbus. Ohio, he!ein refemd to as "Board" and
EMPLOYEE NAME, herein referred to as "Employee", pursua nt to a Resolution duly adopted by the Board on DATE.
WHEREAS. the Superintendent of Schools has recommended the employment of the Employee for not to exceed one (I) year and the
Board has approved such recommendation;
NOW THEREFORE, the parties to this agreement for and in consideration of their mutual promises, do hereby agree as follows:
The Board does hereby enter into a Limited Contract with the Employee for the period commencing DATE and ending on
DATE, for the number of days established by the program work schedule, aod fixes the hourly rate of pay of the Employee at
$$!.$$ per hour. The salary will be paid in accordance with salary schedules adopted by the Board and in effect at the time
service is performed and will be only for services actually performed.
2. The appointment made he!eby is open for acceptance for a period of fifieen ( 15) days from the date of execution by the
Treasurer. Failure to execute and return this agreement within the allotted time will be considered a declination.
J. The appointment made hereby is subject to the right to reassign the Employee whene ver the good of the service may require,
subject to the ~rms of the Master Agreement between the Board of Education and the Columbus Education Association.
4. This individual contract is made pursuant to and subject to all other terms and conditions of the above mentioned Master
Agreement.
5. The Employee agrees to serve the Board of Education of the City School District of Columbus, Ohio, for the aforesaid
period, and further agrees 10 abide by the rules and regulations of the Board now in effect and soch rules and regulations as
may be amended or adopted during the term of this contract, subject to the terms of the Master Agreement.
6. Subject to the provisio ns of Sections 904.07 and 704.03 and Article 401 of the aforementioned Master Agreement, this
contract shall be deemed renewed for a like term at the negotiated salary unless, on or before a date that is ninety (90)
calendar da ys before the ending date of soch co,tract. the Board gives the full·time hourly professional employee employed
there under written notice of its intention not to renew same.
7. The Employee hereby states the following as material representations inducing the Board 10 enter into this contract and
agrees to provide acceptable proof of sa~ prior to commencing service under this contract unless otherwise provided:
(A) istheholderofa validOhioteachingcenificate.
(B) is in a satisfactory state of health and IS free from any communicable disease to the teacher's knowledge.
(C) has provided the Board with an Ohio State Teacher's Retirement System Memhe!ship Record.
(D) has completed and delivered to the Board a Fonn \V-4 for federal income tax purposes ar~ Form IH for Ohio
income tax purposes.
(E) will file with the Board an official transcript of any additional work taken DURING THE PRECEDING SCHOOL
YEAR, INCLUDING SUMMER SCHOOL. NOT LATER THAN SEPTEMBER 15 DURING THE TERM HEREOF.
(F) is not under contract to serve elsewhere during any portion of the term of this conrract.
