One-Year Contract–Home Instruction Tutor

ONE· YEAR HOME INSTRUCTION TUTOR CONTRACT
Agreemem between the Board of Edll(ation of the City School District of Columbus, Ohio, herein referred to as "Board" and
EMPLOYEE NAME, hereinafter referred to as ''futor", pursuant to a Resolution duly adopted by the Board on DATE.
WHEREAS, the Superimeodent of Schools has recommended the employment of the Tutor for the 2008-2009 school year and the Board
has approved such recommendation:
NOW THEREFORE, the panies to this agrument for and in consideration of their mutual promises, do hereby agree as follows:
I. Board does hereby emer imo a One· YearTutorCon1111c1 with the Tutor for the period commencing DATE and coding
on DATE for the number of hours needed and assigned by an appropriate admin~trator, and fixes the hourly rate of pay of the
Employee at $$l.$S per hour as provided in the Master Agreement between the Board and the Columbus Edocation Association. The
salary wilt be paid in accordance with salary schedules adopted by the Board and in effect at the time service is performed and •ill be
only for services actually performed.
2. Aectptanceof appointment shall be made •ithin ll da)' from the date of execution by the Treasurer. Failure to execute and rttum
this agreement within the allotted time will be considered a declination.
3. Tilt appointment made hereby is subject to the right to adjust the assigned hours of work and to reassign the Tutor wtenever the good
of the service may require, subject to the terms of tf.e a hove memioned Master Agreement.
4. This individual contract is made pursuant to and subject to all relevant terms and conditions of the above memioned Master
Agreemem.
i. The Tutor agrees to pro~de senices to the Board of Edoc~ion of the Cny School District of Columbus, Ohio, for the aforesaid
period, and further agre.s to abide by the rules and regulations of the Board now in effect and such rules and regulations as may be
amended or adopted during the term of this contract, subject to the terms of the Ma~er Agreerr£nt.
6. This conll11ct shall be deemed automatically nonrenewed as of its expiration date and no action or notification by the Board shall be
requiredinconnectJOn withsuchnonrenewal.
7. The Tutor hereby s~tes the following as material represen~tions indocing the Board to enter into this contract and agrees to provide
acceptable proof of same prior to commeocing ~ce under this contract unless otherwise pro~ded:
(A) is the holder of a valid Ohio teaching certificate.
(B) is in a satisfactory ~ate of health and is free from any communicable disease to the tutor's knowledge.
(C) has provided the Board with an Ohio S~te Teacher's Retirement System Membership Record.
(D) has completed and delivered to the Board a Form W4 for federal iocome tax purposes and Form IT-4 for Ohio
iocorne tax purposes.
(E) wilt file with the Board an official transcript of any additional work taken DURING THE PRECEDING SCHOOL
YEAR, INCLUDING SUMMER SCHOOL, WITHIN 30 DAYS OF THIS AGREEMENT.
(F) is not under contract to serve elsewhere during any portion of the term of this conlr.lct.
