TEACHER'S LIMITED CONTRACT

WHEREAS, the Superintendent of Schools has recommended the employment of the Teachcr for not to exceed one { l) year and thc Board
has ~ppruvedsuchre. do hereby agree as follows:
I. The Boord does hereby enter imo a Limited Conlrnct with the Teacher for the school year 2009·2010 consisting of 195 days as
provided in the Master Agreement between the Bo:lfd and the Columbus Edocation Associ:uion, and fixes the salai)' of the
Tea.: her at the rate of SiS,SSS pet year, or a proportioMI part of this sum for such portion of the year se rved, in accordance wi th
the sala ry schedule set forth tn the aforementioned Master Agreement as it exists or as it may be amended during the duration of
this contract. Payment shall be made in accordance with one of the pay plans prescribed by the Board as selected by the Teacher.
2. The appointment made hereby is open for acceptance for a period of fifteen (15) days from the date of e~ec ution by the Treasurer
Failure to execute and return this agreement within the allotted time will be considered a declination.
The appointment made hereby is subject to the right to transfer the Teacher from grade to grade, from school to school, and from
one ranking to another, whenever the good of the service may re quire, subject to the terms of the above·mentioned Master
Agreement
4. This individ ual contract is made pursuant to and subjo:t to all other terms and conditions of the above mentioned Master
Agreement.
5. The Tracher agrees to teach in the service of the Board of Education of the City School District of Columbus, Ohio, for the
aforesaid school year, and fu rther agrees to abide by the rules and regulations of the Board now in effect and such rules and
regulations as may be amended or adopted during the term of this contract. subject to the terms of the Master Agreement between
the Board and the Columbus Education Association. The Teacher and the Board agree to abide by all applicable provisions of the
Ohio Revised Code now in effect and such provisions as may be amended or enacted during the term of this contract
6. The Teacher hereby states the following as material representations indu~ing the Board to enter into this cuntract and agrees to
provide acceptable proof of the same prior to commencing service under this contract unless otherwise provided:
(A) is the holder of a valid Ohio teaching certificate
(B) is the holder of a BS, BS including 150 semester boars, or
a MA degree and has submined a complete official
transc ript of teaching training credits to the Board.
(C) is ina satisfactorystate of healthand is freefromany
communicablediseasetotheteacher'sknowledge.
{D) has provided the Board with an Ohio State Teacher's
Retirement System Membership Record
EMPLOYEE NAME Date
(E) Has completed and delivered to the Board a Form W-4 for
federal income tax purpostS and Form IT -4 for Ohio
income tax purposes.
(F) will me with the Board an official llanscri pt of any
additional work taken during the preceding school year,
including summer school, NO LATER THAN SEPTEMBER 15,
