BOARD OF EDUCATION OF THE CITY SCHOOL DISTRICT
COLUMBUS, OHIO
ONE-YEAR TUTOR CONTRACT
Agreement between the Boord of Education of the Ctty School District of Columbus. Ohio, herein referred to as "Board" and
EMPLOYEE NAME, hereafter referred to a~ "Tutor". pursuant to a Resolution duly adopted by the B0.:1rd on DATE.
WHEREAS, the Superintendent of Schools has recommended the employment of the Tutor for the 2008-2009 school year and
the Board hasapprovedsuchrecommencbtion;
NOW THEREFORE, the parties to this agreement for and in consideratloo of their mutual p10mises, do hereby agree as follo·,o,s:

The BQard does hereby enter into a One-Year Tutor Contract .,-jtb the TU!or for the period commencing DATE and ending on
DATE for the number of boars needed and assigned by an appropriate administrator, and ftxes the hourly rate of pay of the
Employee at SSS.SS per hour as provided in the Master Agreeme111 between the Board and the Columbus Edcration
Association. The salary will be paid in accordance with s.:~ lary schedules adopted by the Board and in effecl at the time service
is performed and will be only for services actually performed
The appoimment made hereby is open for acceptance for ~ period of fifteen (IS) days from the date of execution by the
Treasurer. Failure to execute and reiWTithis agreement within the alloued time will beconside1ed a declination.
The appointment made hereby is subject to the right to adjust the assigned hours of wori; and to rea!.Sign the Tutor whenever
the good of the service may require, subject to the terms of the above mentioned Master Agreement
This individual contract is made pursuant tO and subject to all relevant terms and conditions of the above mentioned ~1a.ster
Agreement.
The Tutor agrees to provide services to tile Board of Education of the City School DiSlrict of Columbus, Ohio, for the aforesaid
period, and funher agrees to abide by the rules and regulations of t~.c: Board now in effect and such rules and regula lions as may
be amended or adopted during the term of this contract, subject to the terms of the Master Agreement.
This controct shall be deemed aul0f1\3tically non.renewed as ofits expirationdate and ooaction Of oottfication by the Board

The Tutor hereby states the following as material representations inducing the Board to enter imo this contract and agree~ to

is in a satisfactory $tate of health and is fret from any communicable d~ue to the teacher's knowledge.
has provided the Board ~~oith an Ohio State Teacher's Retirement System Membenhip Record.
has completed and delivered to the Board a Form W-4 for federal income tax purposes and r-orm IT -4 for Ohio income tax
purposes.
will file with the Board an official transcript of any additional wort laken during the preceding school year. including ~t~mmer
school, NOT LATER THAN SEPTEMBER IS DURING THE TERM HEREOF.
is not under oontr3Ct to serve elsewhere during any ponion of the term of tilts contract.
