3.1.2 Submission of London 2012 Paralympic Games accreditation dataApplications for accreditation must be received by LOCOG no later than 20 April2012 in order for NPCs to receive the Pre-Valid Cards before their delegations’departure for the Games.NPCs must complete application forms for all potential participating athletes and NPCteam officials (the long list). This complete list will minimise last-minute changes. NPCswill be able to submit data electronically using LOCOG’s web application(eAccreditation). LOCOG will provide NPCs with a template that NPCs can use toimport applicant data from their existing database.As required by the IPC Handbook, each athlete and NPC team official (all individualsin the A accreditation category including Aa, Ab, Ac, Am, Ao, As, P and NPC horseowners) must complete an IPC Eligibility Code form, acknowledging his/hercompliance with IPC and IPSF rules regarding Games participation. The President orSecretary General must also countersign each form.Eligibility Code forms signed by participating athletes and NPC team officials must bereceived by LOCOG before 20 August 2012. Without a signed form, an athlete willnot be able to compete and NPC team officials will not be eligible for accreditation atthe London 2012 Paralympic Games.
