NEW Related Operational Action <strong>Plan</strong> I.D: Redesign of Student Transition Instruction and Practices<br />

1. The Special Education Task Force will meet to<br />

outline the parameters of the project.<br />

2. Research state and federal regulations, as well as<br />

best practices in <strong>BOCES</strong>, in the region and<br />

statewide. Review and incorporate<br />

facets/opportunities presented by the recent grant<br />

award to Riverhead, Western and Nassau.<br />

3. Discuss and clearly outline key points along the<br />

continuum, and “exit points” (e.g. moving from<br />

elementary to middle school) for students that should<br />

be targets for long term planning efforts of all schools<br />

along the continuum.<br />

4. Develop a comprehensive, multi-year plan that will<br />

address issues identified through the research<br />

process such as:<br />

- An annual skill assessment/exploration that will<br />

address student strengths and interests K-12.<br />

- A continuum of instructional and behavioral skills to<br />

be addressed and integrated into state mandated<br />

curricula.<br />

- A Parent Curriculum to involve them in authentic<br />

assessment and meaningful long term planning for<br />

their child.<br />

- A staff development plan to address staff’s role in<br />

preparing student for “transition”. Special focus will<br />

be on the integration of all service providers in the<br />

planning an implementation of goals and strategies.<br />

5. Communicate the proposed plan to staff<br />

throughout the department to disseminate<br />

information and gather feedback and input from staff<br />

throughout the department.<br />

6. Review staff input and finalize the plan for<br />

implementation.<br />
