demonstr<strong>at</strong>e and document a significant hardship th<strong>at</strong><br />
will impact their ability to continue in the program. Failure<br />
to maintain appropri<strong>at</strong>e progress or achievement in a<br />
course is not sufficient hardship to acquire administr<strong>at</strong>ive<br />
support. The request will be reviewed by the Associ<strong>at</strong>e<br />
Dean <strong>of</strong> Academic Affairs in consult<strong>at</strong>ion with the Office<br />
<strong>of</strong> Student and Pr<strong>of</strong>essional Affairs. Students receiving<br />
administr<strong>at</strong>ive support for withdrawing from the program<br />
will sign a letter <strong>of</strong> agreement from the Associ<strong>at</strong>e Dean<br />
<strong>of</strong> Academic Affairs which st<strong>at</strong>es the reason they are<br />
withdrawing from the program, when they will return to<br />
the program, remedi<strong>at</strong>ion or developmental programs<br />
th<strong>at</strong> must be completed while on leave, and the st<strong>at</strong>us<br />
<strong>of</strong> the student upon returning to the program. Students<br />
withdrawing from the College <strong>of</strong> Pharmacy pr<strong>of</strong>essional<br />
program with administr<strong>at</strong>ive support will be readmitted<br />
to the program upon completion <strong>of</strong> the requirements set<br />
forth in their letter <strong>of</strong> agreement. Students not complying<br />
with their letter <strong>of</strong> agreement will be dismissed from the<br />
program unless the letter has been modified with the<br />
consent <strong>of</strong> the Dean <strong>of</strong> the College <strong>of</strong> Pharmacy or his/<br />
her designee. Students withdrawing from the Doctor <strong>of</strong><br />
Pharmacy Program without administr<strong>at</strong>ive support will<br />
be considered dismissed from the program and must<br />
appeal to the College <strong>of</strong> Pharmacy Academic Standards<br />
Committee for readmission to the program.<br />
Appeals for students who have<br />
WITHDRAWN or been dismissed from the<br />
College <strong>of</strong> Pharmacy. Students who have<br />
withdrawn from the <strong>University</strong> or College without<br />
administr<strong>at</strong>ive support or have been dismissed from the<br />
College <strong>of</strong> Pharmacy for academic reasons may appeal<br />
to the Academic Standards Committee for readmission to<br />
the program. The student should write a letter <strong>of</strong> appeal<br />
to the Chair <strong>of</strong> the Academic Standards Committee<br />
outlining and supporting their request to be readmitted<br />
to the program. The Academic Standards Committee will<br />
review the request and recommend appropri<strong>at</strong>e action to<br />
the Dean <strong>of</strong> the College <strong>of</strong> Pharmacy. The Dean or his<br />
designee will communic<strong>at</strong>e the decision to the student in<br />
question via certified mail.<br />
Students dismissed from the College for pr<strong>of</strong>essional<br />
or ethical reasons, may appeal to the Ethics and<br />
Pr<strong>of</strong>essional Conduct Committee for readmission to the<br />
program. The student should write a letter <strong>of</strong> appeal to the<br />
Ethics and Pr<strong>of</strong>essional Conduct Committee outlining and<br />
supporting their request to be readmitted to the program.<br />
The Ethics and Pr<strong>of</strong>essional Conduct Committee will<br />
review the request and recommend appropri<strong>at</strong>e action to<br />
the Dean <strong>of</strong> the College <strong>of</strong> Pharmacy. The Dean or his<br />
designee will communic<strong>at</strong>e the decision to the student in<br />
question via certified mail.<br />
Students who have withdrawn from the <strong>University</strong><br />
or College without administr<strong>at</strong>ive support or who<br />
have been dismissed from the College for viol<strong>at</strong>ion <strong>of</strong><br />
academic or ethical and pr<strong>of</strong>essional standards will<br />
not be allowed to reapply for admission as a first time<br />
student, and can only be readmitted to the College via<br />
the appropri<strong>at</strong>e committee.<br />

Any decision for readmission to the program may<br />
include conditions <strong>of</strong> remedi<strong>at</strong>ion or development,<br />
including but not limited to repe<strong>at</strong>ing all or part <strong>of</strong> the<br />
program the student has already completed, th<strong>at</strong> must<br />
be successfully completed for the student to reenter or<br />
remain in the program.<br />
DEGREE REQUIREMENTS<br />
In addition to meeting the general requirements<br />
for gradu<strong>at</strong>ion for the <strong>University</strong>, the candid<strong>at</strong>e for the<br />
Doctor <strong>of</strong> Pharmacy degree must, after completing<br />
the requirements <strong>of</strong> the pre-pr<strong>of</strong>essional prepar<strong>at</strong>ory<br />
coursework, be enrolled in the pr<strong>of</strong>essional pharmacy<br />
curriculum for a minimum <strong>of</strong> four academic years<br />
(eight semesters or equivalence) and have earned a<br />
minimum grade <strong>of</strong> “C” in all courses comprising the<br />
degree requirements. Courses graded on a credit<br />
(C), no credit (NC) basis are excluded. In addition to<br />
meeting minimum course grade requirements, students<br />
must meet programm<strong>at</strong>ic requirements for gradu<strong>at</strong>ion<br />
which may include but not be limited to <strong>at</strong>tendance<br />
<strong>at</strong> student convoc<strong>at</strong>ions, particip<strong>at</strong>ion in pr<strong>of</strong>essional<br />
development programs and successful completion <strong>of</strong><br />
progression and/or competency exams. A majority <strong>of</strong><br />
semester hours required in the pr<strong>of</strong>essional program must<br />
be earned <strong>at</strong> ULM. All advanced practice experiences<br />
must be completed through ULM using ULM approved<br />
practitioners and practice sites. Finally, students must<br />
demonstr<strong>at</strong>e the pr<strong>of</strong>essional m<strong>at</strong>urity and demeanor<br />
necessary to succeed in the pr<strong>of</strong>ession <strong>of</strong> pharmacy.<br />
COMPUTER LITERACY<br />
Computer literacy for pharmacy majors is defined as<br />
an ability to effectively utilize represent<strong>at</strong>ive commercially<br />
available pharmacy applic<strong>at</strong>ions s<strong>of</strong>tware in pharmacy<br />
practice. Each student must demonstr<strong>at</strong>e computer<br />
literacy. Students entering the College <strong>of</strong> Pharmacy<br />
pr<strong>of</strong>essional program should be well versed in common<br />
computer applic<strong>at</strong>ions such as electronic mail, internet<br />
applic<strong>at</strong>ions, word processing, present<strong>at</strong>ion s<strong>of</strong>tware,<br />
and d<strong>at</strong>a management s<strong>of</strong>tware such as MS Excel(R)<br />
and MS Access(R). Students who have never used a<br />
personal computer for common applic<strong>at</strong>ions such as<br />
word processing are encouraged to complete Computer<br />
Science 1070 (or equivalent) prior to enrolling in the<br />
pr<strong>of</strong>essional pharmacy program.
