may not be awarded once the Bachelor’s degree<br />

or higher is earned.<br />

4. A second degree may not be awarded in General<br />

Business if the first degree is in any other field in<br />

business.<br />

REQUIREMENTS FOR THE SECOND<br />

MAJOR<br />

Students who wish to earn a second major may do<br />

so by successfully completing all courses required for<br />

the second major as specified in this c<strong>at</strong>alog, provided<br />

both the primary and secondary majors lead to the<br />

same baccalaure<strong>at</strong>e degree (e.g., B.A., B.B.A., B.S.).<br />

Supporting courses will not be required. Work for the<br />

second major must be <strong>of</strong> the same quality as th<strong>at</strong> required<br />

for the first. As required by the Board <strong>of</strong> Regents, a<br />

student's diploma will reflect only the degree design<strong>at</strong>ion<br />

(e.g., Bachelor <strong>of</strong> Science.) The degree design<strong>at</strong>ion, as<br />

well as both majors, will appear on the student's transcript.<br />

REQUIREMENTS FOR A MINOR<br />

Requirements for a minor for those students pursuing<br />

bachelor degrees may be found under “Re quire ments<br />

for a Minor” in the Courses <strong>of</strong> Instruction section in this<br />

c<strong>at</strong>alog. Students pursuing associ<strong>at</strong>e degrees cannot earn<br />

a minor. Minor subject areas are not posted on a transcript<br />

subsequent to the completion <strong>of</strong> a bachelor’s degree.<br />

STUDENT HEALTH SERVICES POLICY<br />

Student Health Services<br />

It is well known th<strong>at</strong> college students have special<br />

health needs. ULM Student Health Services recognizes<br />

these needs and stands ready to provide inform<strong>at</strong>ion<br />

and confidential tre<strong>at</strong>ment for these problems, as well<br />

as general medical services for all students. Student<br />

Health Services is also available to address any <strong>of</strong> your<br />

immuniz<strong>at</strong>ion compliance questions and needs.<br />

Student Health Services is loc<strong>at</strong>ed in the Student<br />

Health and Counseling Center <strong>at</strong> the corner <strong>of</strong> <strong>University</strong><br />

and LaSalle Streets and is staffed by Board Certified<br />

Family Nurse Practitioners. Clerical personnel are<br />

available to handle your billing and referral needs. An<br />

immuniz<strong>at</strong>ion specialist is also available to answer your<br />

immuniz<strong>at</strong>ion compliance questions. Call (318) 342-5238<br />

for your immuniz<strong>at</strong>ion questions.<br />

General Inform<strong>at</strong>ion<br />

Student Health Services functions on a walk-in<br />

basis. Students are encouraged to visit the clinic during<br />

non-class time. Appointments are required for women’s<br />

health wellness visits. You may be provided a follow-up<br />

appointment <strong>at</strong> the discretion <strong>of</strong> your nurse practitioner.<br />

Students failing to cancel a scheduled appointment will be<br />

assessed a no-show fee.<br />

Student I.D. cards are required for service. No<br />

excuses will be provided for missed classes. Call (318)<br />

Current Services<br />

Board certified nurse practitioners can provide many<br />

types <strong>of</strong> services and tre<strong>at</strong>ments for students such as:<br />

• Screening and tre<strong>at</strong>ment <strong>of</strong> sexually transmitted<br />

diseases<br />

• HIV Testing<br />

• Immuniz<strong>at</strong>ions<br />

• Allergy injections<br />

• Non-narcotic prescriptions and over the counter<br />

medic<strong>at</strong>ions<br />

• Women’s health<br />

• Mental health<br />

• Minor surgical procedures<br />

• Family health sickness or wellness visits<br />

• Complete labor<strong>at</strong>ory services<br />

• Physical exams<br />

• Nutritional Counseling<br />

• Smoking Cess<strong>at</strong>ion<br />

Hours <strong>of</strong> Oper<strong>at</strong>ion<br />

Student Health Services is open Monday through<br />

Thursday from 7:30 a.m. to 5:00 p.m. and on Friday from<br />

7:30 a.m. to 11:30 a.m. In the event <strong>of</strong> an emergency<br />

students should dial 1-911 on campus and 911 <strong>of</strong>f<br />

campus. During non-oper<strong>at</strong>ional hours, students should<br />

seek medical tre<strong>at</strong>ment with their personal physician or <strong>at</strong><br />

any <strong>of</strong> the local emergency rooms or walk-in clinics.<br />

Student Fees<br />

Most medical services will be covered by your<br />

university assessed student health fee. Additional fees<br />

may be incurred for some procedures deemed necessary<br />

by your practitioner. Questions about fees owed and<br />

instructions for filing your medical claims are available.<br />

A fee schedule is posted in Student Health Services for<br />

your viewing. Fees incurred are the responsibility <strong>of</strong> the<br />

student. Students are expected to pay when services<br />

are rendered. Failure to pay student health fees within<br />

30 days will result in suspension <strong>of</strong> your student health<br />

service privileges until your account is in good standing.<br />

Failure to pay fees within 60 days will result in flagging <strong>of</strong><br />

your student account. Flagging <strong>of</strong> your account will carry<br />

the same consequences as other <strong>University</strong> debt flags.<br />
