o PA 6029
Information reported in Section I by the
participant and Section II by the landlord
regarding address, landlord, and rent amount
must match. If there is a discrepancy, or the
information is not completed, the PA 6029 will
be stamped as “Incomplete / Discrepancy
Found” with incomplete discrepant areas
highlighted in yellow and returned to the DR
EW via county mail using a PA 26-1,
Miscellaneous Transmittal.
o W-9
The W-9 must include the landlord’s name,
address, TIN, signature and date. If the form
is incomplete, it will be voided and returned to
the landlord with a new W-9 and a postagepaid
envelope pre-addressed to the
CalWORKs Program central site staff. The
landlord must complete the new W-9 form
and mail it back to the central site staff. The
Direct Rent process cannot continue until the
W-9 is complete.
NOTE: A new W-9 must be completed when
information on the form must be
corrected.
Validates the TIN on the IRS website per
existing procedures when the W-9 is completed
correctly. If the landlord’s TIN cannot be
validated, the DR EW will be notified via FAC.
The invalid W-9 will be returned to the landlord
stamped “Invalid TIN.”
NOTE: The W-9 is not returned to the district
office.
Once the landlord’s TIN has been validated:
o Checks the box “Tax ID/SSN Validated” on
the PA 6029; and
o Forwards the PA 6029 and W-9 forms via
County mail with a PA 26-1, Miscellaneous
Transmittal to the DPSS Special Operations
Section (SOS).
