The application is to be printed out, completed
and faxed/mailed to the phone number/address
provided on the application.

The applicant/participant is required to return the
completed CW 2201 or EDD verification to the EW.
Upon return of the completed document, the
applicant/participant is deemed to have met this
condition of eligibility.
How should the applicant/participant be referred
to the Employment Development Department
(EDD) to apply for Disability Insurance Benefits
(DIB)?
Once it is determined that the applicant/participant
is “apparently eligible” to DIB, the EW should direct
the applicant/participant to apply for DIB by using
one of the following methods:
• Telephone - call the EDD toll-free telephone
number between 8 a.m. and 5 p.m. Monday
through Friday at:
o English 1-800-480-3287
o Spanish 1-866-658-8846
o All others 1-800-480-3287

• Apply on-line via the internet at:
http://www.edd.ca.gov
