• If the information is blank, the
applicant/participant must fill in the
information or must write “unknown” if the
information is not available.
• If the names are cut-off/incomplete or the
applicant/participant makes an error, he/she
must draw a line (no white out), initial and
write in the correct information.
• The form must be completed manually (out
of drawer) by Non-English/Non-Spanish
speaking participants. CW 2.1Qs not
generated by LEADER must be thoroughly
reviewed to ensure all questions have been
accurately completed.
• The completed CW 2.1Q is stored in the
case record for future audit purposes.
What is an electronic child support referral?
LEADER sends CCSAS case and persons
information including absent parent, participant
and child information. This is considered an
electronic child support referral. LEADER will
send this information to CCSAS when:
• CalWORKs benefits are authorized for a
case with an absent parent(s) – Intake cases
• Absent parent is added to LEADER –
Approved cases
CCSAS will generate a task for CSSD staff to
open a child support case. The
applicant/participant will be contacted to
complete a child support interview over the
telephone. If a telephone number is not
available (on LEADER), CSSD staff will contact
the applicant/participant by mail.
