• Verification of the AU’s MAP exempt status;
• Maximum Family Grant (MFG) informing notice or
other documentation verifying that MFG informing
requirements have been met;

• Verification of age-appropriate immunization;
• Copies of any documents supporting the eligibility
determination;
• IEVS Applicant System abstract/printout; and
• Notices of Action (NOA) for current or future
overpayment/overissuance collection.
How are ICTs handled when the case involves a victim
of Domestic Violence (DV)?
In instances when an ICT is known in advance, the
counties must work together to ensure that information
regarding DV services, waivers, and security issues are
transferred as seamlessly as possible to avoid
endangering the family and to reduce duplication of
verification for the family who is transferring to another
county.
The WTW 37 – Permission to Release Domestic Abuse
Information When Moving to Another County form (out of
drawer) must be provided to the recipient when she/he
self-declares or when it is known that the participant is
receiving DV services. At a minimum, the WTW 37
should be provided at application and at each subsequent
waiver review meeting with an explanation that she or he
has the right to terminate the release at any point.
When LA County is the sending county, the SSS EW/SSS
GSW should provide the participant with references to
local DV services that are located in the receiving county.
These services can be found by calling 1-800-799-SAFE
and/or by contacting the receiving county. The DV
victim’s safety is of paramount concern; thus, strict
confidentiality must be maintained. All communication of
DV issues, including documentation regarding DV
services and waivers, must be kept confidential. This
information can only be released if the recipient has
signed a WTW 37.
