Проведена оценка возможно<strong>с</strong>ти эндо<strong>с</strong>копиче<strong>с</strong>кого гемо<strong>с</strong>таза <strong>с</strong>амора<strong>с</strong>ширяющими<strong>с</strong>я нитиноловыми <strong>с</strong>тентами Даниша<br />
в каче<strong>с</strong>тве альтернативы баллонной тампонаде зондом-обтуратором Сенг<strong>с</strong>такена ― Блэкмора в комплек<strong>с</strong>ной лечебной<br />
программе больных <strong>с</strong> о<strong>с</strong>трыми кровотечениями из варикозно ра<strong>с</strong>ширенных вен пищевода. Пред<strong>с</strong>тавлены техниче<strong>с</strong>кое<br />
у<strong>с</strong>трой<strong>с</strong>тво <strong>с</strong>тента Даниша, методики его у<strong>с</strong>тановки и удаления, а также анализ <strong>с</strong>об<strong>с</strong>твенного опыта лечения 19 пациентов<br />
на вы<strong>с</strong>оте пищеводного кровотечения или <strong>с</strong> вы<strong>с</strong>оким ри<strong>с</strong>ком рецидива по<strong>с</strong>леднего у<strong>с</strong>тановкой <strong>с</strong>амора<strong>с</strong>ширяющих<strong>с</strong>я<br />
нитиноловых <strong>с</strong>тентов Даниша. К преимуще<strong>с</strong>твам <strong>с</strong>тента Даниша <strong>с</strong>ледует отне<strong>с</strong>ти малую травматично<strong>с</strong>ть, хорошую<br />
перено<strong>с</strong>имо<strong>с</strong>ть пациентом, обе<strong>с</strong>печение физиологиче<strong>с</strong>кого дренажа <strong>с</strong>люны и возможно<strong>с</strong>ти приема жидко<strong>с</strong>ти и пищи через<br />
рот, повторный эндо<strong>с</strong>копиче<strong>с</strong>кий о<strong>с</strong>мотр пищевода и желудка по<strong>с</strong>ле введения <strong>с</strong>тента, <strong>с</strong>нижение ри<strong>с</strong>ка а<strong>с</strong>пирационной<br />
пневмонии, невозможно<strong>с</strong>ть удаления или <strong>с</strong>мещения <strong>с</strong>амим пациентом в <strong>с</strong>о<strong>с</strong>тоянии возбуждения. К недо<strong>с</strong>таткам <strong>с</strong>тентов<br />
Даниша, помимо ри<strong>с</strong>ка миграции из пищевода в про<strong>с</strong>вет желудка <strong>с</strong> потерей <strong>с</strong>воей тампонирующей функции, <strong>с</strong>ледует отне<strong>с</strong>ти<br />
необходимо<strong>с</strong>ть ане<strong>с</strong>тезиологиче<strong>с</strong>кой поддержки при у<strong>с</strong>тановке <strong>с</strong>тента, вы<strong>с</strong>окую вероятно<strong>с</strong>ть повторного кровотечения<br />
по<strong>с</strong>ле удаления <strong>с</strong>тента, ограничение лечебного эффекта только варикозно ра<strong>с</strong>ширенными венами пищевода<br />
и развитие мелких эрозий или язв на <strong>с</strong>лизи<strong>с</strong>той оболочке пищевода по<strong>с</strong>ле удаления <strong>с</strong>тента. Эндо<strong>с</strong>копиче<strong>с</strong>кий гемо<strong>с</strong>таз<br />
<strong>с</strong>амора<strong>с</strong>ширяющими<strong>с</strong>я нитиноловыми <strong>с</strong>тентами Даниша у пациентов <strong>с</strong> кровотечениями из ВРВ пищевода можно ра<strong>с</strong><strong>с</strong>матривать<br />
как разумную альтернативу баллонной тампонаде зондом-обтуратором Сенг<strong>с</strong>такена ― Блэкмора.<br />

Danis stent for esophageal bleeding: for and against<br />

The possibility of endoscopic hemostasis with self-expanding nitinol Danis stents as an alternative to balloon Sengstaken ― Blackmore<br />
tamponade in the complex treatment program of patients with acute bleeding from varicose esophageal veins. The own clinical experience<br />
is presented in treating 19 patients with high esophageal bleeding or high risk of its recurrence by self-expanding nitinol stent<br />
installation instead of the traditional Danish balloon Sengstaken ― Blackmore tamponade. The advantages of Danish stent include<br />
low trauma; good tolerance by the patient; providing the physiological drainage of saliva and the possibility of taking liquids and food<br />
through the mouth; repeated endoscopic inspection of the esophagus and stomach, after the introduction of the stent; reducing the risk<br />
of aspiration pneumonia; the impossibility of removal or displacement by the patient in a state of excitement. The disadvantages of selfexpanding<br />
nitinol Danis stents, in addition to the risk of migration from the esophagus into the lumen of the stomach with loss of its plugging<br />
function, include the need for anesthesia during stent installation, and high probability of repeated bleeding after removal of the<br />
stent, and limitation of therapeutic effect only with varicose dilated esophagus veins, and development of small erosions or ulcers on<br />
the mucous membrane of the esophagus after the stent removal. Endoscopic hemostasis by self-expanding nitinol Danis stents can be<br />
regarded as a reasonable alternative to balloon Sengstaken ― Blackmore tamponade in patients with varicose esophageal bleeding.<br />
