Проанализированы причины неудовлетворительных отдаленных результатов паллиативных операций при ки<strong>с</strong>тозной<br />
тран<strong>с</strong>формации желчных протоков (21 больной) и у 2-х пациентов по<strong>с</strong>ле радикального и<strong>с</strong><strong>с</strong>ечения ки<strong>с</strong>т гепатикохоледоха.<br />
При паллиативных операциях, типа ци<strong>с</strong>тодиге<strong>с</strong>тивных ана<strong>с</strong>томозов, не удаляют<strong>с</strong>я ки<strong>с</strong>тозноизмененные протоки, не<br />
у<strong>с</strong>траняют<strong>с</strong>я патогенетиче<strong>с</strong>кие факторы заболевания (рефлюк<strong>с</strong>, хрониче<strong>с</strong>кое во<strong>с</strong>паление, холе<strong>с</strong>таз, малигнизация), а<br />
имевшие ме<strong>с</strong>то до операции клиниче<strong>с</strong>кие проявления (рецидивирующий холангит, желтуха и др.) о<strong>с</strong>тают<strong>с</strong>я, что диктует<br />
необходимо<strong>с</strong>ть выполнения повторной операции, которая заключает<strong>с</strong>я в мак<strong>с</strong>имальном и<strong>с</strong><strong>с</strong>ечении измененных <strong>с</strong>тенок<br />
ки<strong>с</strong>ты и протоков <strong>с</strong> формированием широкого гепатикоэнтероана<strong>с</strong>томоза по Ру. По<strong>с</strong>ле радикального и<strong>с</strong><strong>с</strong>ечения ки<strong>с</strong>ты<br />
желчных протоков в отдельных <strong>с</strong>лучаях может развить<strong>с</strong>я <strong>с</strong>триктура гепатикоэнтероана<strong>с</strong>томоза при трудно<strong>с</strong>тях<br />
ана<strong>с</strong>томозирования и нарушении правил прецизионной техники. Повторные операции у таких больных заключают<strong>с</strong>я в<br />
рекон<strong>с</strong>трукции <strong>с</strong>оу<strong>с</strong>тья.<br />

The reasons were analyzed for unsatisfactory remote results of palliative operations of cyst transformations of biliary ducts<br />
(21 patients) and 2 patients after extensive excision of hepatico-choledochal cysts. Palliative operations like cytodigestive anastomoses<br />
do not remove the cyst-transformed ducts, and the pathogenic factors of the disease are not eliminated (reflux, chronic inflammation,<br />
choleostasis, malignization), while the pre-operative clinic manifestations (recurrent cholangitis, jaundice, etc) preserve, which<br />
determines the necessity for the recurrent operation with maximal excision of the transformed duct and cyst walls and forming the wide<br />
hepaticoenteroanamostosis by Rue. After the extensive excision of hepatico-choledochal cysts a stricture of hepaticoenteroanamostosis<br />
may sometimes develop, due to complicated anamostosing and breaches of precision technique. Recurrent operations in such patients<br />
include reconstruction of the anamostosis.<br />
