L'activité hématopoïétique est souvent conservée, au moins à un stade initialde la maladie, et la cellularité est normale (sur les trois lignées médullaires) sinonaccrue. On peut noter une hypocellularité avec une diminution de la granulopoïèseet de l’érythropoïèse [17].La lignée mégacaryocytaire est quasiment toujours hyperplasique au débutavec une maturation qui s’effectue correctement [7], la lignée rouge apparaît parfoisdysplasique. La lignée granuleuse, préservée à la phase initiale de la maladie, peutdevenir déplétive au cours de l’évolution [14].Une érythroblastose est fréquente, témoin de l’érythropoïèse réactionnelle àl’hémolyse intramédullaire [36].Le médullogramme peut être normal ce qui n’élimine pas le diagnostic [7],il doit alors être à nouveau réalisé. Il est pour certains plus rentable que la biopsiemédullaire [78] Cependant, il doit être lu par un cytologiste averti et orienté par leclinicien car les images de phagocytose sont parfois rares. Une hémophagocytoseintramédullaire peut se rencontrer au cours d’hémolyse ou d’autres affectionshématologiques.Le contexte étiologique peut parfois être évoqué ou confirmé sur lemyélogramme lorsqu'il existe un infiltrat hémopathique en particulierlymphomateux, ou lorsqu'il est mis en évidence des signes d'activation lymphoïdequi peuvent faire évoquer une pathologie virale ou une lymphohistiocytose [14,88].A.2 : Autres examens cytologiques :Il est possible, de façon très rare, de retrouver des signes d'hémophagocytosedirectement dans le sang périphérique [140]. Des examens cytologiques serontrarement effectuées etpeuvent révéler l’hémophagocytose active mais restentmoins performants: la cytoponction ganglionnaire, la recherche d’hémophagocytose134
