Solvent <str<strong>on</strong>g>Abuse</str<strong>on</strong>g> Trends in Japan<br />

RECENT TRENDS IN SUBSTANCE ABUSE IN JAPAN<br />
<str<strong>on</strong>g>Nati<strong>on</strong>al</str<strong>on</strong>g> epidemiologic data from household surveys <strong>on</strong> substance abuse<br />
are unavailable in Japan. However, the number <str<strong>on</strong>g>of</str<strong>on</strong>g> arrests for substance<br />
c<strong>on</strong>trol law violati<strong>on</strong>s is <strong>on</strong>e <str<strong>on</strong>g>of</str<strong>on</strong>g> the indicators that suggest the magnitude<br />
<str<strong>on</strong>g>of</str<strong>on</strong>g> the problem. Figure 1 presents the number <str<strong>on</strong>g>of</str<strong>on</strong>g> arrests for the violati<strong>on</strong>s<br />
against various substance c<strong>on</strong>trol laws from 1950 to 1988. In the case <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
organic solvents. pers<strong>on</strong>s diverted to treatment also are included in the<br />
number, as most <str<strong>on</strong>g>of</str<strong>on</strong>g> the solvent abusers are under the age <str<strong>on</strong>g>of</str<strong>on</strong>g> 20.<br />
The numbers <str<strong>on</strong>g>of</str<strong>on</strong>g> abuse cases involving marijuana and heroin are relatively<br />
low as reflected in medical emergencies. Only a few cases resulting from<br />
those substances were treated in hospitals in recent years throughout the<br />
country. However, arrests for violati<strong>on</strong>s against the Cannabis C<strong>on</strong>trol<br />
Law were substantial in number. There is a discrepancy between the<br />
number <str<strong>on</strong>g>of</str<strong>on</strong>g> arrests for Cannabis C<strong>on</strong>trol Law violati<strong>on</strong>s and the magnitude<br />
<str<strong>on</strong>g>of</str<strong>on</strong>g> resultant social and medical problems. In c<strong>on</strong>trast to marijuana and<br />
heroin, the magnitude <str<strong>on</strong>g>of</str<strong>on</strong>g> abuse <str<strong>on</strong>g>of</str<strong>on</strong>g> organic solvents and methamphetamine<br />
have been uniquely high and c<strong>on</strong>tinuous since the early 1970s. In 1988,<br />
data compilati<strong>on</strong> methods <str<strong>on</strong>g>of</str<strong>on</strong>g> the <str<strong>on</strong>g>Nati<strong>on</strong>al</str<strong>on</strong>g> Police Agency changed, and<br />
the figure no l<strong>on</strong>ger included individuals diverted to treatment for solvent<br />
abuse (figure 2). During the next 2 years, the trend reflects a c<strong>on</strong>tinuati<strong>on</strong><br />
<str<strong>on</strong>g>of</str<strong>on</strong>g> the high plateau.<br />
Table 1 shows the number and percentage <str<strong>on</strong>g>of</str<strong>on</strong>g> psychiatric patients with<br />
various types <str<strong>on</strong>g>of</str<strong>on</strong>g> substance abuse problems from 1976 to 1991. In Japan,<br />
stimulant abuse almost exclusively involves methamphetamine. For<br />
unique reas<strong>on</strong>s, cocaine is classified in the same group with morphines,<br />
both legally and administratively. Patients who are treated in psychiatric<br />
hospitals tend to have a l<strong>on</strong>g history <str<strong>on</strong>g>of</str<strong>on</strong>g> abuse, become dependent, and<br />
exhibit mental symptoms. Psychiatrists observed an epidemic in<br />
psychiatric hospitals admissi<strong>on</strong>s for solvent abuse between the mid- and<br />
late 1970s. In 1976, patients with solvent abuse accounted for 17.7<br />
percent <str<strong>on</strong>g>of</str<strong>on</strong>g> all substance abuse patients. In 1981, the percentage rose to<br />
41.5 percent and still c<strong>on</strong>tinues at a high level. The proporti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
methamphetamine abuse also is around 40 percent, similar to that <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
solvent abuse (see stimulants <strong>on</strong> table 1).<br />
