I. End of project report or terminal report on the project summarizing the
recommendations and activities undertaken in the project including
documentations and compilation of prototype materials to be submitted in 6 hard
copies and 6 electronic copies stored in compact discs

PROJECT SITES
Project sites will include Central Office and CHDs Nationwide. The branding of behavior
and creative materials will be initially used in 150 urbanized cities and provinces but it
will also be use in the roll out sites in year 2 and 3 of the project.
