MANUAL PROCEDURES FOR THE PROCUREMENT OF CONSULTING SERVICES<br />

IV. SCHEDULE OF PAYMENT<br />

The following payment scheme shall be applied <strong>for</strong> this<br />

consultancy:<br />

A. Fifteen percent (15%) <strong>of</strong> <strong>the</strong> total amount shall be<br />

given to <strong>the</strong> consultant upon submission and approval<br />

<strong>of</strong> <strong>the</strong> inception report<br />

B. Thirty percent (30%) <strong>of</strong> <strong>the</strong> balance shall be released<br />

upon submission and approval <strong>of</strong> <strong>the</strong> first draft<br />

C. Thirty percent (30%) <strong>of</strong> <strong>the</strong> balance shall be released<br />

upon approval and acceptance <strong>of</strong> <strong>the</strong> outputs by <strong>the</strong><br />

cluster head and executive committee<br />

D. Twenty-five percent (25%) <strong>of</strong> <strong>the</strong> balance shall be<br />

released upon submission <strong>of</strong> <strong>the</strong> final end-<strong>of</strong>-<strong>the</strong>project<br />

report and <strong>the</strong> creative outputs<br />

V. TIME FRAME OF ENGAGEMENT<br />

The work shall be completed within six (6) month from<br />

date <strong>of</strong> issuance <strong>of</strong> Notice to Proceed (NTP).<br />

VI. IMPLEMENTING ARRANGEMENTS<br />

A. Project Management<br />

Person W will manage <strong>the</strong> contract with <strong>the</strong> assistance<br />

<strong>of</strong> Person X, Chief Health Communications Division<br />

and Person Y, OIC, Health Program Promotion<br />

Division<br />

B. Reporting Obligations<br />

1. The Contracting party shall submit to <strong>the</strong><br />

Department <strong>of</strong> Health through <strong>the</strong> National Center<br />

<strong>for</strong> Health Promotion <strong>the</strong> final prototype creative<br />

materials, produced copies <strong>of</strong> some materials<br />

reports and o<strong>the</strong>r relevant documents specified in<br />

<strong>the</strong> TOR as agreed upon during negotiation, six (6)<br />

hard copies and six (6) s<strong>of</strong>t copies stored in CD<br />

while <strong>for</strong> drafts <strong>of</strong> prototypes and reports one (1)<br />

each <strong>for</strong> print and s<strong>of</strong>t copy.<br />

2. The Inception Report shall have <strong>the</strong> following<br />

minimum contents:<br />

a. Activities, resource requirements <strong>for</strong> each<br />

activity and a PERT-CPM Chart<br />

b. Background<br />

c. Objectives<br />

d. Implementation Strategy<br />

e. Activities<br />

f. Personnel and Resource Requirement (describe<br />

<strong>the</strong> personnel and resource requirement <strong>of</strong> <strong>the</strong><br />

Project and how <strong>the</strong>y are to be deployed to<br />

effectively pursue <strong>the</strong> objective<br />

g. Manning schedule<br />

h. Work Plan. The report shall contain a<br />

GHANTT chart showing relationships among<br />

interrelated activities. The work plan shall<br />

indicate workloads <strong>for</strong> <strong>the</strong> TA team<br />

i. The Consultant shall prepare and submit to <strong>the</strong><br />

<strong>DOH</strong> materials or documents <strong>for</strong> <strong>the</strong> <strong>DOH</strong>’s<br />

approval or review through <strong>the</strong> <strong>DOH</strong> Project<br />

Manager<br />
