3. A Progress reporting shall be done on monthly basis, which includes:<br />
a. Objective(s) <strong>for</strong> <strong>the</strong> period<br />
b. Accomplishments or assessment <strong>of</strong> progress including justification<br />
<strong>of</strong> changes from <strong>the</strong> work plan and <strong>the</strong> implications<br />
c. Departures from <strong>the</strong> work plan; cumulative deviations to date from<br />
schedule <strong>of</strong> progress milestones as specified in <strong>the</strong> final project<br />
plan<br />
d. Corrective actions to be taken to return to planned schedule <strong>of</strong><br />
progress; proposed revisions to work schedule<br />
e. O<strong>the</strong>r issues and outstanding problems; proposed actions to be<br />
taken<br />
f. Resource that producer expects to be provided by <strong>DOH</strong> and/or<br />
actions to be taken by <strong>DOH</strong> <strong>for</strong> <strong>the</strong> next reporting period<br />
g. O<strong>the</strong>r issues or potential problems <strong>the</strong> Contractor <strong>for</strong>esees that<br />
could impact on project progress and/or effectiveness<br />
h. Payments; Payments requested and received will be included in <strong>the</strong><br />
report, along with a statement <strong>of</strong> cumulative payments.<br />
4. Approval/Review <strong>of</strong> Technical Documents and O<strong>the</strong>r Outputs by <strong>the</strong> <strong>DOH</strong><br />
a. The first draft <strong>of</strong> <strong>the</strong> prototype materials shall be submitted to <strong>the</strong><br />
project manager <strong>of</strong> <strong>the</strong> NCHP<br />
b. The project manager will facilitate <strong>the</strong> presentation and <strong>the</strong><br />
subsequent approval <strong>of</strong> all prototype creative outputs initially by<br />
<strong>the</strong> SMCT and PSDT Cluster members and <strong>the</strong> final approval by<br />
<strong>the</strong> EXECOM members <strong>of</strong> <strong>the</strong> <strong>DOH</strong> preferably within 15 days<br />
c. If <strong>the</strong> creative outputs were disapproved or need some<br />
modifications, <strong>the</strong> consultant shall modify <strong>the</strong> outputs and resubmit<br />
it <strong>for</strong> <strong>DOH</strong>’s approval prior to production <strong>of</strong> some materials<br />
d. The Consultant will submit to <strong>the</strong> National Center <strong>for</strong> Health<br />
Promotion <strong>the</strong> following reports:<br />
Progress reports – a monthly or update report <strong>of</strong> activities taken by<br />
<strong>the</strong> Contractor including prototype materials approved.<br />
Final report – end <strong>of</strong> project detailing all activities being undertaken<br />
or specified in <strong>the</strong> Scope <strong>of</strong> Work <strong>of</strong> <strong>the</strong> Terms <strong>of</strong> Reference. The<br />
final report shall include <strong>the</strong> communication executional strategy,<br />
<strong>the</strong> branding <strong>of</strong> <strong>the</strong> behavior, prototypes developed, PR plan, launch<br />
event plan, pre- test results and <strong>the</strong> final creative materials.<br />
