3. Progress reporting shall be done on monthly basis,<br />
which includes:<br />
a. Objective(s) <strong>for</strong> <strong>the</strong> period<br />
b. Accomplishments or assessment <strong>of</strong><br />
progress including justification <strong>of</strong> changes<br />
from <strong>the</strong> work plan and <strong>the</strong> implications<br />
c. Departures from <strong>the</strong> work plan; cumulative<br />
deviations to date from schedule <strong>of</strong> progress<br />
milestones as specified in <strong>the</strong> final project<br />
plan<br />
d. Corrective actions to be taken to return to<br />
planned schedule <strong>of</strong> progress; proposed<br />
revisions to work schedule<br />
e. O<strong>the</strong>r issues and outstanding problems;<br />
proposed actions to be taken<br />
f. Resource that producer expects to be provided<br />
by <strong>DOH</strong> and/or actions to be taken by <strong>DOH</strong><br />
<strong>for</strong> <strong>the</strong> next reporting period<br />
g. O<strong>the</strong>r issues or potential problems <strong>the</strong><br />
Contractor <strong>for</strong>esees that could impact on<br />
project progress and/or effectiveness<br />
h. Payments; Payments requested and<br />
received will be included in <strong>the</strong> report, along<br />
with a statement <strong>of</strong> cumulative payments<br />
4. Approval/Review <strong>of</strong> Technical Documents and<br />
o<strong>the</strong>r Outputs by <strong>the</strong> <strong>DOH</strong><br />
a. The first draft <strong>of</strong> <strong>the</strong> prototype materials shall<br />
be submitted to <strong>the</strong> project manager <strong>of</strong> <strong>the</strong><br />
NCHP<br />
b. The project manager will facilitate <strong>the</strong><br />
presentation and <strong>the</strong> subsequent approval <strong>of</strong> all<br />
prototype creative outputs initially by <strong>the</strong><br />
SMCT and PSDT Cluster members and <strong>the</strong><br />
final approval by <strong>the</strong> EXECOM members <strong>of</strong><br />
<strong>the</strong> <strong>DOH</strong> preferably within 15 days<br />
c. If <strong>the</strong> creative outputs were disapproved or<br />
need some modifications, <strong>the</strong> consultant shall<br />
modify <strong>the</strong> outputs and resubmit it <strong>for</strong> <strong>DOH</strong>’s<br />
approval prior to production <strong>of</strong> some materials<br />
d. The Consultant will submit to <strong>the</strong> National<br />
Center <strong>for</strong> Health Promotion <strong>the</strong> following<br />
reports:<br />
Progress reports – a monthly or update<br />
report <strong>of</strong> activities taken by <strong>the</strong><br />
Contractor including prototype materials<br />
approved.<br />
Final report – end <strong>of</strong> project detailing all<br />
activities being undertaken or specified in<br />
<strong>the</strong> Scope <strong>of</strong> Work <strong>of</strong> <strong>the</strong> Terms <strong>of</strong><br />
Reference. The final report shall include<br />
<strong>the</strong> communication executional strategy,<br />
<strong>the</strong> branding <strong>of</strong> <strong>the</strong> behavior, prototypes<br />
developed, PR plan, launch event plan,<br />
pre-test results and <strong>the</strong> final creative<br />
materials.<br />
