Dear Mr. V,
This pertains to the Hiring of an Advertising Agency for the Branding of Behavior, Development and
Production of Creative/Campaign Materials for the Breastfeeding Communication for Behavioral Impact
(COMBI) under REI-CS-GOP-2010-08 wherein your company expressed its intention to participate in the
bidding.
During the opening of eligibility documents held on 28 June 2010, the Bids and Awards Committee (BAC)
found that your firm failed to submit the Audited Financial Statements, showing, among others, the
consultant‟s total and current assets and liabilities, stamped “received” by the BIR or its duly accredited and
authorized institutions, for the preceding calendar year which should not be earlier than two (2) years from
the date of bid submission. Hence, your firm was declared “INELIGIBLE”. As provided for in Section 55.1 of
Republic Act No. 9184 and its Revised Implementing Rules and Regulations, you have three (3) calendar
days upon receipt of this notice within which to file your Motion for Reconsideration.
