5. Provide ongo<strong>in</strong>g (bi-monthly m<strong>in</strong>imum) monitor<strong>in</strong>g of the mentor-mentee relationshipsand provide consultation and assistance to all parties. BBBS will keep a record of the monitor<strong>in</strong>gcontacts.6. Measure the outcomes for the mentee at the end of the school year. Outcomemeasurement will be performed us<strong>in</strong>g Youth Outcome Survey (YOS), which is a mentor<strong>in</strong>goutcome measurement <strong>in</strong>strument. (A copy of this YOS is attached to this MOU.)7. Provide reports regard<strong>in</strong>g outcomes to the Board and/or Pr<strong>in</strong>cipals at the end of theschool year.8. Provide a copy of each pre-approved BBBS Memorandum of Understand<strong>in</strong>g signed by<strong>in</strong>dividual schools.9. Work to carefully <strong>in</strong>corporate the mentor<strong>in</strong>g program outcomes <strong>in</strong>to the Board’s goals forstudents.10.a. BBBS agrees to <strong>in</strong>demnify, defend and hold harmless the Board’s and its respectiveelected/appo<strong>in</strong>ted officials, agents, employees, volunteers, donors, and sponsors from any and allclaims, demands, suits, and actions, <strong>in</strong>clud<strong>in</strong>g reasonable attorney’s fees, litigation expenses andcourt costs, connected therewith, aris<strong>in</strong>g as a result of any direct or <strong>in</strong>direct, willful, or negligentact or omission of BBBS or its officers, agents, employees, or volunteers, except to the extentcaused by the negligence of Board, <strong>in</strong>clud<strong>in</strong>g Board’s officers, employees, and agents.b. The obligation to <strong>in</strong>demnify, defend, and hold the Board harmless shall rema<strong>in</strong> <strong>in</strong> fullforce and effect follow<strong>in</strong>g the term<strong>in</strong>ation of this MOU until all statutes of limitations haveexpired.11. Neither BBBS or any of its employees or agents shall be considered to be an employee ofthe Board for any purpose.12. BBBS and its employees, agents, volunteers, and mentors shall ma<strong>in</strong>ta<strong>in</strong> theconfidentiality of all student records (see paragraph B.l. below) <strong>in</strong> compliance with federal andstate laws.Section B:The School Board of Sem<strong>in</strong>ole <strong>County</strong>, Florida will:1. Upon receipt of the necessary permission forms (see paragraph A.3. above), provideBBBS access to student grades, attendance and suspension records for program evaluation.2.
