If you are present during ANY patient/consumer interaction at a health fair or screening, you:<br />

MUST wear your <strong>Pfizer</strong> name tag <strong>and</strong> clearly identify yourself as a <strong>Pfizer</strong> employee; <strong>and</strong><br />

MUST NOT offer any medical opinions, advice or consultation even if you have a license to<br />

practice medicine or are any other type of HCP.<br />

Securing Consent <strong>and</strong> Personal Information from Consumers<br />

As a general policy, <strong>Pfizer</strong> does not use Personal Information to communicate directly with patients<br />

unless the patient has consented (or “opted in”) to receiving such communications.<br />

<strong>Pfizer</strong> has detailed guidelines for all of our permitted activities that involve the collection <strong>and</strong> use of<br />

patients’ personal information to ensure compliance with all applicable laws <strong>and</strong> <strong>Pfizer</strong> policies. These<br />

activities include, but are not limited to:<br />

Disease management program enrollment forms;<br />

Coupons <strong>and</strong> rebate offers;<br />

Literature requests;<br />

Loyalty programs; <strong>and</strong><br />

Health screenings.<br />

These guidelines apply only when consumers are asked to provide Personal Information, such as name,<br />

address, e-mail address or phone number. When a <strong>Pfizer</strong> program requires a consumer to provide<br />

Personal Information as a term or condition of use of or access to the program, a simple, timely,<br />
