Providing Your Personal Information When Required By Vendor Credentialing<br />

As you have seen, <strong>Pfizer</strong> is committed to protecting the privacy of patients’ <strong>and</strong> customers’ Personal<br />

Information. <strong>Pfizer</strong> is also committed to protecting your privacy <strong>and</strong> the privacy of other colleagues<br />

from inappropriate use by or disclosure to third parties. Moreover, <strong>Pfizer</strong> also wants to ensure that<br />

when colleagues’ information is entrusted to third parties, it is properly protected from unauthorized<br />

disclosure. <strong>Pfizer</strong>’s Institutional Access <strong>Guide</strong>lines demonstrate this commitment to colleagues <strong>and</strong><br />

their privacy. These <strong>Guide</strong>lines can be found on PfieldNet under the “Compliance” tab.<br />

More <strong>and</strong> more hospitals <strong>and</strong> health care institutions are conditioning site access on colleagues’<br />

submission of Sensitive Personal Information <strong>and</strong> compliance with other vendor credentialing<br />

requirements. Often the stated purpose of these submissions <strong>and</strong> requirements is to make sure that<br />

people with access to personnel, patients <strong>and</strong> visitors do not have serious communicable illnesses or a<br />

history of violent acts. Required Personal Information can include immunization status, copies of<br />

medical records demonstrating inoculation or immunity to certain illnesses, whether you have had a<br />

background check (<strong>and</strong> its outcome), your training history <strong>and</strong> your professional qualifications.<br />

<strong>Pfizer</strong> respects the hospital’s or vendor’s desire to secure the health <strong>and</strong> safety of its personnel, patients<br />

<strong>and</strong> visitors. Accordingly, the Institutional Access <strong>Guide</strong>lines were created. In particular, <strong>Pfizer</strong> has<br />

created a Vendor Credentialing team to help you respond to these requests. You can find the team’s<br />

contact information on PfieldNet at http://PfieldNet.pfizer.com, under the “Compliance” tab.<br />

Regardless of whether a customer or institution asks for your Personal Information directly or indirectly<br />

(e.g., through a vendor hired to collect data on behalf of the customer), <strong>Pfizer</strong> wants your privacy to be<br />

respected <strong>and</strong> your Personal Information appropriately protected. Here are some key points for you to<br />

remember:<br />

Always tell your manager if a hospital or institution (or institution’s vendor) wants you to<br />

provide your Personal Information to gain site access.<br />

If the hospital or vendor has a written credentialing policy, be sure to provide a complete<br />

<strong>and</strong> current copy to the Vendor Credentialing team <strong>and</strong> your Regional Attorney to review in<br />

advance.<br />
