Give the hospital or vendor <strong>Pfizer</strong>’s approved template Confidentiality Letter Agreement<br />

(available through a link in the <strong>Guide</strong>lines, accessible on PfieldNet at<br />

http://PfieldNet.pfizer.com).<br />

Do not share your Personal Information before the health care provider <strong>and</strong>/or vendor signs<br />

an approved Confidentiality Agreement to protect your information.<br />

Do not modify <strong>Pfizer</strong>’s approved template Confidentiality Letter Agreement without your<br />

Regional Attorney’s approval.<br />

Do not sign a non-<strong>Pfizer</strong> Confidentiality Agreement without your Regional Attorney’s<br />

approval.<br />

<strong>Pfizer</strong> Regional Attorneys will review relevant hospital <strong>and</strong> HCP policies to ensure that any<br />

agreements are acceptable for you to sign <strong>and</strong> do not pose potential legal issues for <strong>Pfizer</strong>. Regional<br />

Attorneys will review the agreements in light of <strong>Pfizer</strong>’s interests <strong>and</strong> cannot offer you personal legal<br />

advice regarding your personal privacy or other concerns. Once the Regional Attorney has approved<br />

an agreement, it is your responsibility to carefully read <strong>and</strong> underst<strong>and</strong> it because you will be held<br />

accountable by the institution for compliance with it. Violations of an institution’s policies may lead<br />

to <strong>Pfizer</strong> being denied the ability to visit or hold programs at that institution.<br />

Corporate Policies on Your Responsibility for Safeguarding Personal Information<br />

You should familiarize yourself with the following <strong>Pfizer</strong> corporate policies <strong>and</strong> guides:<br />

Corporate Policy 404 on Protecting the Privacy of Personal Information;<br />

Corporate Policy 403 on the Acceptable Use of <strong>Pfizer</strong> Information Systems; <strong>and</strong><br />

Corporate Policy 405 on Enterprise Records <strong>and</strong> Information.<br />

These documents provide important guidance about appropriate information h<strong>and</strong>ling <strong>and</strong> security<br />

procedures, which include, but are not limited to:<br />

Not leaving your <strong>Pfizer</strong> equipment or Personal Information unattended or in an unsecured<br />

location, e.g., an unlocked car;<br />

Encrypting your computer <strong>and</strong> using encrypted USB flash drives;<br />
