Organizing and <strong>Design</strong>ing<br />

<strong>Questionnaire</strong>s<br />

Since the first edition of this book was published, there have been<br />

substantial changes in the way surveys are conducted. Although<br />

many surveys are still traditionally conducted over the telephone or<br />

in person with paper and pencil, an increasing percentage of surveys<br />

are conducted by interviewers with computer assistance. This<br />

includes Web-based surveys, e-mailed questionnaires for large surveys,<br />

and one-on-one interviews conducted with personal digital<br />

assistant (PDA) devices. Because researchers use traditional paperand-pencil<br />

methods and also computer-assisted approaches, this<br />

chapter is split into three parts: (1) a discussion of general formatting<br />

issues that apply to both methods, (2) a discussion of formatting<br />

issues related to computer-assisted interviews, and (3) a<br />

discussion of formatting issues related to paper-and-pencil surveys.<br />

General Formatting Issues<br />

Why is formatting important? <strong>The</strong> format of a questionnaire determines<br />

how easy it is for interviewers, respondents, and data processing<br />

personnel to read and understand the questions and the<br />

kind of answers required. Thus, the quality of the data becomes<br />

heavily influenced by the questionnaire format (Sanchez, 1992). A<br />

general principle to follow in formatting is that the respondent’s<br />

needs must receive top priority, the interviewer’s needs next highest<br />

priority, and the data processing staff’s needs the lowest priority<br />

(since they are not subject to the stresses of the interview). Ideally,<br />

the questionnaire should be formatted to meet all of these needs<br />
