Another threat is the laboratory staff shortage. Declining student interest in the field of laboratory medicine has been highlighted, particularly in the United States. A recent report shows an impressive decline in clinical laboratory science programs producing medical technologists between 1975 and 2005, resulting in fewer and fewer graduates each year. The personnel shortage affects not only medical technologists, but also medical graduates entering pathology specialties. The dichotomization between the clinical world and the isolated production of laboratory test results represents a fracture of the brain-to-brain loop and also results in ineffective care associated with an uncoordinated patient journey through the health care process. A path for the future should take into consideration the need of a comprehensive view of laboratory medicine as an academic discipline and of clinical laboratories as source of information, not only analytical results. Laboratuvar tıbbının kalite güvencesine, standart geliştirmeye ve performans takibine yoğunlaşan uzun bir geçmişi vardır. Bu geçmiş, hata riskini azaltma ve hasta güvenliğini geliştirme için iyi bir temel oluşturur. Son on yılda laboratuvar tıbbında hatalara yaklaşım büyük ölçüde değişmiş, sadece analitik hataların önemsendiği “laboratuvar merkezli” bakıştan sıyrılmış, toplam test sürecindeki hatalara odaklanan “hasta merkezli” bakışa yönelmiştir. Güncel kanıtlar, analiz öncesi ve sonrası aşamaların analitik aşamaya göre çok daha hataya açık olduğunu göstermektedir. Ek olarak, test döngüsünün başlangıcı ve sonu araştırıldığında, “prepre” ve “post-post” analitik süreçlerin ne klinik laboratuvar içinde yürütüldüğü, ne de kısmen bile olsa laboratuvar personeli tarafından control edildiği, ama hataya daha açık olduğu ortaya çıkmıştır. Benzer şekilde, son birkaç on yılda, hasta güvenliği kavramı da evrim geçirmiş ve önemi artmıştır, ancak sağlık alanındaki hatalarla ilgili varolan verilerin çoğu, özellikle de hastanelerdeki ilaç tedavisi kaynaklı hatalara odaklanmıştır ve advers olaylarla ilintilidir. Bununla birlikte, son birkaç yılda yapılan büyük ölçekli araştırmalar göstermiştir ki, gerek hastalar, gerekse hekimler tanısal hataların yaygınlığı ve öneminin farkındadırlar. Gerçekte son on yıl içinde tanısal hatalar Birleşik Devletler’deki hatalı uyugulama (malpraktis) iddiaları içinde en yaygın hata tipi haline gelmiştir. Tanı süreci, aslında çok sayıda klinik aşama, çok sayıda tedarikçi ve çok sayıda arızadan kaynaklanan ve hastaya zarar veren hatalar içerir ve bunlar katkı yapan etkenler olarak kesişirler. Laboratuvar tıbbında hatalar tanısal hatalarda önemli rol oynar ve bu durum laboratuvarcıların daha çok gayret göstermesi anlamına gelir. Mario PLEBANI Department of Laboratory Medicine, University-Hospital, Padova-Italy The journey towards quality and patient safety in laboratory medicine represents a major challenge as interventions should be multifactorial and complex, target multiple persons - including laboratory professionals, care teams and patients - and use various incentives and levers. Laboratory medicine has a long history of careful attention to quality assurance, standard setting and performance monitoring. This is an important foundation to build upon for reducing the risk of errors and improving patient safety. In the last decade, the approach to errors in laboratory medicine has been strongly modified, moving from a “laboratory-centered” scenario which recognised only analytical errors, to a “patientcentered” scenario that focus on errors in the total testing process. A milestone in this process is a consensually accepted taxonomy for “errors in laboratory medicine” which has been developed and defined in an ISO Technical Specification (ISO/TS 22367: 2008). Current evidence demonstrates that pre-and post-analytic steps are much more vulnerable to errors than the analytic phase. In addition, on exploring the beginning and the end of the testing loop, it emerges that currently these steps -“pre-pre”- and “post-post”-analytic steps- performed neither in the clinical laboratory nor, at least in part, under the control of laboratory personnel, are more error-prone than others. Analogously, in the last few decades, the concept of patient safety evolved and emerged to assume an increasing importance, but most available data on errors in health care focus on medication-related errors, particularly in hospitals, and related adverse events. However, in the last few years large-scale surveys demonstrated that patients and physicians perceive that diagnostic errors are common and of concern. As a matter of fact, throughout the last decade, diagnostic errors have become the most prevalent type of malpractice claim in the United States. The diagnostic process, in fact, consists of numerous clinical steps, stretched across multiple providers, and errors that could harm patients result from the alignment of multiple breakdowns, which in turn stem from a confluence of contributing factors. Errors in laboratory medicine play a relevant role in the context of diagnostic errors and call for new efforts by laboratory professionals. The path towards safety and quality in laboratory medicine has moved from the analytical phase to a patient-centered scenario in which laboratory professionals have to take care of all steps and procedures performed from test ordering to results interpretation and this should be done only through teamwork and interdisciplinary co-operation.
