Özen AKYÜREK Ventura Yazılım, Cyber Plaza A Blok No: 202 06533 Bilkent/Ankara, Türkiye ozen@ventura.com.tr Tıbbi laboratuvarlarda bilgi teknolojileri hastanın değil de hastaya ait örneğin taşınabilirliği, testlerin çeşitliliği ve çok istenen tetkiklerin büyük oranda otomatik cihazlarla çalışabilirliği nedeni ile en başından etkin ve yaygın olarak kullanılmaktadır. Barkod teknolojilerinin kullanımının tıp alanında yaygınlaşması da aynı şekilde her zaman laboratuvara paralel olmuştur. Bununla birlikte laboratuvarlarda RFID (Radio Frequency IDentification) ve 2D barkod (2 Dimensional, Data Matrix, Karekod) kullanımı en azından Türkiye için yaygın durumda değildir. Laboratuvar cihazlarının henüz RFID ve 2D barkodları okuma özelliğine sahip olmadığını, bu sebeple de laboratuvarlarda kullanılamayacağını söylemek, laboratuvarlarda kullanılan bilgi sistemlerini cihazlara istem gönderen ve cihazlardan istem sonuçlarını alan sistemler olarak görmek olacaktır. Oysa bu teknolojilerin kullanımı sayesinde: numunenin gerçekten de örnek alınması gereken hastadan alınıp alınmadığı ve ne zaman alındığı; örnekler laboratuvara gelene kadar hangi sıcaklıklara maruz kaldığı; laboratuvara ne zaman geldiği (geleneksel olarak teker teker barkod okutmak dışında); herhangi bir anda fiziksel olarak nerede olduğu; yeniden erişmemiz gereken örneklerin hangi dolabın hangi lokasyonunda olduğunu bilmek mümkündür. Son yıllarda tıbbi laboratuvarlarda maliyetlerin düşürülmeye çalışılması nedeni ile merkezileşme eğilimi söz konusudur. Artık birçok tetkikin örneğin alındığı yerde değil, daha düşük maliyetle ve daha güvenli sonuçların üretilebildiği diğer laboratuvarlarda çalışılmasından söz etmek mümkündür. Birçok küçük laboratuvar ve hastane (hatta bazı büyük hastaneler) aldığı örnekleri fiziksel olarak merkezi laboratuvarlara taşıyabilmekte, ancak bilginin elektronik ortamda merkezi laboratuvara taşınmasında ve sonuçların merkezi laboratuvardan alınmasında sorunlar yaşanmaktadır. IHE ve LOINC standartlarının dünya genelinde kabul görmesinin ardından, bu standartları kullanan 2D barkodların laboratuvarlar arası bilgi alış verişini standardize etmesinden söz edilebilmektedir. Eskiden tüplerin üzerine markör kalemle hasta ismi yazılır, istem kağıdıyla birlikte laboratuvara gönderilirdi. Özen AKYÜREK Ventura Software, Cyber Plaza A Blok No: 202 06533 Bilkent/Ankara, Turkey ozen@ventura.com.tr Information Technologies have been widely and effectively used in medical laboratories due to a number of reasons. First of all, samples, unlike patients, are portable. Secondly, it is automatic devices that run frequently ordered tests. Also, there is a wide range of tests ordered. Similarly, the usage of bar-code technology in medicine in general has always been parallel to its use in laboratories in particular. However, the current practice shows us that RFID (Radio Frequency Identification) and 2D Bar-code (2 Dimensional, Data-matrix) applications are not common in Turkey. Should one think that laboratory devices are incapable of reading RFID and 2D bar-codes and thus are unsuitable for laboratories, it is similar to assuming that laboratory information systems are limited to sending orders to devices and receiving the test results. Yet, thanks to the use of RFID and 2D technologies, laboratories gain a number of advantages. They allow us to make sure that the sample was drawn from the patient in question as well as the time of bloodletting. They tell us which temperatures the samples have been exposed to. They show when these samples reached the laboratory (instead of the traditional practice of reading several separate bar-codes one by one). Also, we know where each sample is or was at a specific time. And finally, it is possible to detect the exact location of the necessary sample at a specific refrigerator later. In the past few years because of the attempt to decrease the costs, there has been a tendency for laboratories to become centralized. Nowadays, it is common to run the tests at a place where the costs are lower and the obtained results are reliable rather than at a place where actual bloodletting was done. Although many small laboratories and hospitals (even big ones) can carry the samples to centralized laboratories, they have difficulties transferring the information there and receiving the result data later in electronic format. Since IHE and LOINC standards are widely accepted all over the world, 2D bar-codes generated in accordance with these standards are most likely to standardize the information exchange between laboratories. In the past, patient names used to be written on the tube with a marker and sent to the laboratory together with the order form. Bu da sağlık hizmeti sunucularına, bilgi teknolojisi sağlayıcılarından bile bağımsız olarak, diledikleri tetkikleri diledikleri yerde çalıştırmak gibi müthiş bir esneklik sağlıyor. Üstelik bu teknoloji sonuçların sağlık hizmeti sunucusunun bilgi sistemine aktarılmasında da kullanılmaya hazır. Tek eksik LOINC kodlama sisteminin de (TC Kimlik, ICD10 ve SUT gibi) Sağlık Kodları Referans Sunucusu’na (SKRS) entegre edilmesi. 2D ve RFID teknolojileri yakın zamanda kaliteli sonuçları uygun maliyetle üretebilen laboratuvarları, birkaç adım öne çıkaracak gibi görünüyor. Toplamda 3230 karakterden oluşan bu özetin 2 adet 2D barkoda (şimdilik 2335 karakterle sınırlı olan) sığabiliyor olması belki de bunun en önemli göstergesi. This innovation allows healthcare providers to be super flexible, even independent from IT providers, while ordering the tests of their choice at a place of their choice. Moreover, this technology is ready for transferring the result data to the information system server of the healthcare provider. The only missing element is the integration of LOINC into SKRS (Turkish Health Codes Reference Server) like ICD10, Turkish Identity Number and SUT (Turkish Health Budget Law). It seems that in the near future, 2D and RFID technologies will bring the reliable and affordable result - producing laboratories to the fore. The fact that this summary, which consists of 3460 characters, fits into 2 data-matrix bar-codes (currently limited to 2335 characters), can be considered the best evidence of what has been said.
