Son zamanlarda otomatize kan sayım cihazları hücresel analizlerdeki yeni fiziksel prensipler ve bilgisayar yazılımlarındaki gelişmelerle birlikte teknolojik bir evrim geçirmiştir. Sonuç olarak analitik verimlikte ve elde edilen bilgide artış sağlanmıştır. Fonksiyonel demir eksikliği anemisinde kullanılan yeni parametreler yanında çekirdekli eritrositler (NRBC), olgunlaşmamış granülositler (IG), olgunlaşmamış retikülositler (IRF), olgunlaşmamış trombositler (IPF) ve eritrosit parçaları gibi çok sayıda yeni parametre kan sayımına eklenmiştir. Daha önce sadece anormal hücre alarmı şeklinde gösterilen bazı hücreler artık sayılabilir hale gelmiştir. Rutin analizörlerden biri monoklonal antikor kullanarak akım sitometrisi ile hücre sayımı yapabilmektedir. Laboratuvar uzmanları ve klinisyenlerin kan sayımı metodları ve yeni parametreler konusundaki gelişmeleri takip etmeleri gerekmektedir. İdeal olarak bu parametreler farklı üretici cihazları arasında standardize edilmelidir, ancak bu yeni parametreler için henüz akredite bir dış kalite kontrol programı dahi yoktur. Ayrıca bazı parametreler için bir grup analizörde iç kalite kontrolü de bulunmamaktadır. Otomatik analizörlerde bulunan fakat henüz tam potansiyellerine ulaşamamış yeni analitik parametrelerin olası klinik kullanımı ile ilgili daha ileri incelemelere ihtiyaç vardır. Olgunlaşmamış retikülosit fraksiyonu (IRF), ortalama retikülosit hacmi (MCVr) ve ortalama retikülosit hemoglobin içeriği (CHr) gibi retikülosit indeksleri, parçalanmış eritrositlerin sayısı (FRBC) ve olgunlaşmamış trombosit fraksiyonu (IPF) bunların arasındadır. Ortalama trombosit hacmi (MPV) ve trombosit dağılım genişliği (PDW) gibi bazı parametrelerin de dikkatli kullanılması gereklidir. Bu parametreler yıllardır kan sayım cihazlarında bulunmasına rağmen yine de eritrosit dağılım genişliği (RDW) kadar standardize değildir. Otomasyonun önemli rolüne rağmen patolojik örneklerin mikroskobik kontrolünün kaçınılmaz olduğu unutulmamalıdır. Ayrıca kullanılan cihazın sınırlamalarının bilinmesi sonuçların doğru yorumu için çok önemlidir. NEW PARAMETERS IN BLOOD CELL COUNTING AND TECHNICAL PROBLEMS Nurzen SEZGİN Department of Medical Biochemistry, Faculty of Medicine, Baskent University, Adana The complete blood count is the most requested parameter of laboratory tests. Recently automated blood cell counters have undergone a technological evolution owing to the introduction of new physical principles for cellular analysis and the progressive evolution of software. The results have been an improvement in analytic efficiency and an increase in information provided. Several parameters have been introduced to the complete blood count such as nucleated red blood cells (NRBC), immature granulocytes (IG), immature reticulocyte fraction (IRF), immature platelet fraction (IPF) and red cell fragments as well as new parameters for detection of functional iron deficiency. Some abnormal cells that were previously only indicated to be present in the blood by the generation of an abnormal cell flag are now quantified. One manufacturer’s routine analyzer allows measurement of cells by flow cytometry using monoclonal antibodies. Both laboratory scientists and clinicians need to keep up to date with new parameters and methods in hematology. Ideally these parameters should be standardized between different manufacturer’s instrument, however there are currently no accredited external quality assesment schemes (EQAS) for these new parameters. Additionally for some of the parameters, on a number of instruments, there is no internal quality control available. Further considerations are necessary regarding the possible clinical use of new analytical parameters that are available only with automated analyzers but that have not yet reached their full potential. The immature reticulocyte fraction (IRF), reticulocyte indices such as mean reticulocyte volume (MCVr) and mean reticulocyte hemoglobin content (CHr), fragmented RBC (FRBC) count, and the immature platelet fraction (IPF) are among these. Other parameters, such as the mean platelet volume (MPV) and platelet distribution width (PDW), must be used with caution. Despite being available for several years, they are still not standardized as RBC distribution width (RDW). It should be remembered that despite the essential role of automation, microscopic control of pathologic samples remains indispensable. Moreover, knowledge of the limits of the specific analyzer in use is important for the correct interpretation of results.
