Serum total bilirubin ölçümü yenidoğanlarda en sık yapılan testlerin başında<br />
gelir. Hemen hemen her yenidoğan için en az bir defa bilirubin ölçümü yapılır.<br />
Ölçümler için tercih edilebilecek yöntemler başlıca 3 grupta toplanabilir: 1)<br />
Transkutanöz bilirubin seviyelerini ölçen noninvaziv hastabaşı test cihazları<br />
(POCT), 2) Kan örneklerinde direkt fotometrik ya da spektrofotometrik ölçüm<br />
yapabilen bilirubinometreler, 3) Serum ya da plazmada kimyasal reaksiyon ile<br />
total bilirubin seviyelerini spektrofotometrik olarak ölçen laboratuar analizörleri.<br />
Kullanımı giderek yaygınlaşan transkutanöz bilirubinometreler, kan örneği<br />
gerektirmediği için hızlı ve pratik bir şekilde bilirubin ölçümü yapar. Ölçüm prensibi<br />
olarak yenidoğan cildine cihaz tarafından gönderilen spesifik dalga boylarındaki<br />
ışığın deri ve deri altında biriken bilirubin tarafından emilmesi ve absorbanstaki<br />
değişikliğin cihaz tarafından bilirubin konsantrasyonuna çevrilmesine dayanır.<br />
Alından ya da daha iyisi sternum alt ucundan ölçüm yapılabilir. Dezavantajları,<br />
belirli gestasyonel yaş aralığında kullanılabilmesi ve 20 mg/dL’ye kadar lineer<br />
sonuçlar vermesidir. Birçok yöntem karşılaştırma çalışmasında transkutanöz<br />
bilirubinometrelerin güvenilir olmakla birlikte olması gerekenden daha düşük<br />
sonuçlar verdiği ve bilirubin seviyeleri yükseldikçe negatif yöndeki farkın arttığı<br />
belirtilmiştir.<br />
Kan örneklerinde direkt spektrofotometrik yöntemi kullanan bilirubinometreler,<br />
yenidoğan bilirubin ölçümlerinde günümüzde en çok tercih edilen cihazlardır.<br />
Bu grup içinde çoğunluğu hematokrit pipetinde kapiller kan örneğinin santrifüj<br />
sonrası plazmadan direkt fotometrik yöntem ile ölçüm yapan cihazlar oluşturur<br />
(Bilimeter-3, Erma B-105N gibi). Bu tür cihazları elde etmenin ucuz ve kolay<br />
olması, düşük örnek hacmi, ölçüm aralığının geniş olması (0-30 mg/dL), saniyeler<br />
içinde sonuç alınması, gestasyonel yaş ya da doğum ağırlığından etkilenmeme<br />
gibi avantajları vardır. Nadir olarak kan gazı analizörlerinde (ABL Radiometer ya<br />
da Omni S Roche gibi) santrifüje gerek olmadan tam kan örneğinde diğer önemli<br />
parametreler ile birlikte total bilirubin ölçümü de yapılmaktadır. Bu yöntemde<br />
düşük örnek hacmi ile çabuk sonuç elde edilebilmesine rağmen maliyet yüksektir.<br />
Serum ya da plazmada kimyasal reaksiyon ile total bilirubin seviyelerinin<br />
spektrofotometrik ölçümü referans yöntemdir. İzlenebilirlik, tekrarlanabilirlik<br />
ve lineeritesinin yüksek olması avantajları, diğer yöntemlere göre daha fazla kan<br />
The most frequent laboratory test performed in the newborns is total serum<br />
bilirubin measurement. Almost at last once determination of bilirubin performs<br />
for each newborn. The preferable methods for this purpose can essentially be<br />
grouped as 1) point-of-care devices for noninvasive reading of the skin to measure<br />
transcutaneous bilirubin (TcB) levels, 2) bilirubinometers for nonchemical direct<br />
photometric or spectrophotometric measurement in blood samples, 3) laboratory<br />
analyzers for measurement of total bilirubin levels in serum or plasma after a<br />
chemical reaction.<br />
Routine use of transcutaneous bilirubinometers becomes widespread because<br />
it doesn’t depend on blood sample. They measure quick and practical way of<br />
bilirubin levels. The spesific multiple wavelength light is transmitted to the<br />
infant skin from TcB device. It’s absorbed from the bilirubin at and under the<br />
skin than bilirubin concentrations are estimated by measuring the alteration of the<br />
absorbances. Determinations can be obtained from the infants’ forehead and the<br />
lower end of the sternum. Using between the certain gestational age and giving<br />
linearity results up to 20 mg/dL are disadvantages of this device. There are a lot of<br />
studies that compare different methods about bilirubin measurement for neonates.<br />
However transcutaneous bilirubinometers give reliable results, according to these<br />
studies they usually give underestimated results especially increasing discrepancy<br />
at higher TcB values.<br />
Bilirubinometers that use direct spectrophotometric methods in blood samples<br />
are most preferable devices for bilirubin measurement of newborns. In this group,<br />
majority of devices compose direct photometric method devices that measure<br />
from plasma after centrifugation of capillary blood sample in hematocrit tube<br />
(Bilimeter 3, Erma B-105N, etc). Advantages of these devices are low cost, easy<br />
attainable and low sample volume. Furthermore they have wide measurement<br />
range (0-30 mg/dL) and give results in seconds. Birthweight and gestational age<br />
don’t affect these devices. Blood gas analyzers (ABL Radiometer, Omni S Roche)<br />
don’t necessitate centrifugation of the blood sample. They can measure serum<br />
total bilirubin with the other important parameters. These devices give results<br />
quickly with small volume of blood sample but costs are usually high.<br />
