fascitis plantar crónica puede asociarse con nódulos
fibrosos dolorosos, mientras que los fibromas plantares
no son dolorosos.
La fibromatosis plantar es una afección benigna, localmente
agresiva, con proliferación de fibroblastos en el
TCS de la planta del pie. Ella predomina en las porciones
medial y distal de la aponeurosis, a diferencia de lo que
ocurre en la fascitis plantar. En la ecografía se puede ver
una masa nodular hipoecoica. Se le conoce también como
enfermedad de Ledderhose.
La ruptura de la fascia plantar es rara, casi siempre
localizada cerca de su inserción en el calcáneo y puede ser
aguda o crónica.
La ecografía tiene valor en estas entidades para lo cual
se requieren transductores de muy alta resolución.
LESIONES TUMORALES ÓSEAS Y DE LAS PB
El tobillo y el pie pueden ser asiento de múltiples lesiones
tumorales óseas y de las PB. Entre las lesiones óseas
benignas más frecuentes están los encondromas y los tumores
benignos de células gigantes. Estas articulaciones
pueden ser asiento de cualquier variedad de tumor maligno,
pero son muy raras las lesiones metastásicas, así como
las lesiones en el curso de los linfomas y la leucemia.
El valor de la ecografía en estos casos es el tratar de
diferenciar entre las lesiones quísticas y sólidas y auxiliados
del Doppler color, analizar las características
vasculares de las mismas, sobre todo de las lesiones de la
PB o cuando las lesiones óseas rompen la cortical y se
extienden por las regiones vecinas.
