TCL1 and CD11c expression in hairy-cell<br />
leukemia and their diagnostic role on bone<br />
marrow biopsies in the differential diagnosis<br />
with splenic marginal zone lymphoma<br />

Background. According to WHO classification, hairy cell<br />
leukaemias (HCL) always express CD22, CD103 and CD11c;<br />
it is also typically positive for CD25, TRAP, DBA.44 and<br />
FMC7. On fixed tissues, however, no routinely available single-marker<br />
is specific for HCL, since CD11c, CD22 and even<br />
TRAP and DBA. 44 can be present in other malignancies,<br />
such as splenic marginal zone lymphoma (SMZL), that may<br />
morphologically and clinically mimic HCL. Annexin A1<br />
(ANXA1) immunocytochemical detection has demonstrated<br />
to be highly sensitive and specific for HCL cells on peripheral<br />
blood and on formalin fixed tissues, but its expression in<br />
myeloid cells can create difficulty in staining interpretation 1 .<br />
TCL1 (T-cell leukemia 1 gene) is an oncogene involved in<br />
chromosome rearrangements in mature T-cell leukemia. In<br />
B-cell lymphomas, evaluated by immunohistochemistry,<br />
TCL1 expression has been documentated in B-cell neoplasms<br />
of pre-GC and GC origin, but not in marginal zone<br />
lymphoma and myeloma, and its reactivity pattern in HCL is<br />
currently not described 2 . We investigated TCL1 and CD11c<br />
immunoreactivity in 10 cases of HCL (formalin-fixed paraffin-embedded<br />
bone marrow biopsies), comparing the results<br />
of TCL1 immunostaining in 10 cases of SMZL, in order to<br />
evaluate its possible role in the differential diagnosis of the<br />
two entities.<br />
Methods. Immunohistochemistry was performed using high<br />
temperature antigen retrieval in citrate buffer (pH 8) for 30<br />
min on deparaffinized sections. Monoclonal antibodies<br />
recognising TCL1 (clone 27D6/20, dilution 1:500; MBL, Naka-ku<br />
Nagoya, Japan) and to CD11c (clone 5D11, dilution<br />
1:50; Novocastra Laboratories, Newcastle, United Kingdom)<br />
were used with a polymeric labelling two-step method (Super<br />
sensitivet IHC detection system, Biogenex, San Ramon, CA,<br />
USA) in an automated staining system (GenoMx i6000, Biogenex).<br />
Results. All investigated HCL cases were CD11c and TCL1<br />
positive (10/10). Hairy cells showed a moderate, focally intense<br />
membrane staining for CD11c. 9 cases showed moderate<br />
or intense nuclear/cytoplasmic staining for TCL1; in 1<br />
TCL1 was detected (weak staining) only on a variable fraction<br />
of neoplastic cells. No SMZL stained with TCL1 (0/10).<br />
Conclusion. Both TCL1 and CD11c show a high sensitivity<br />
for HCL cells on formalin-fixed paraffin-embedded bone<br />
marrow biopsies. TCL1 negativity in SMZL may be useful in<br />
the differential diagnosis.<br />

Introduzione. I meccanismi biologici che regolano l’angiogenesi<br />
non sono stati analizzati in maniera approfondita nelle<br />
Ph - MPC. La quasi totalità degli studi ha messo in evidenza<br />
un incremento della DMV nelle Ph - MPC ed in particolare<br />
nella mielofibrosi idiopatica cronica (CIMF). In queste malattie<br />
non è chiaro il ruolo del VEGF, principale fattore proangiogenetico<br />
e i dati in letteratura sono discordanti. Questo<br />
studio ha lo scopo di valutare la densità microvascolare<br />
(DMV) e l’espressione immunoistochimica di VEGF nelle<br />
biopsie osteomidollari (BOM) di pazienti affetti da Ph - MPC.<br />
Metodi. La popolazione esaminata comprende 98 pazienti<br />
(60 M e 38 F; M/F = 1,6/1; età media: 61 aa., range: 18-85<br />
aa.) di cui 29 con Trombocitemia Essenziale (TE), 30 con Policitemia<br />
Vera (20 in fase policitemica e 10 in fase mielofibrotica)<br />
e 39 con CIMF (11 CIMF-0, 11 CIMF-1, 7 CIMF-2,<br />
10 CIMF-3). I casi controllo (CC) sono rappresentati da 20<br />
BOM di stadiazione, prive di alterazioni istologiche.<br />
La DMV è stata valutata mediante anticorpo anti-CD34, utilizzando<br />
due differenti metodiche: il metodo “hot-spots” e il<br />
metodo “semi-quantitativo”.<br />
L’espressione immunoistochimica di VEGF è stata valutata<br />
come VEGF index, definito come la cellularità midollare<br />
moltiplicata per la frazione di cellule VEGF-positive, ed<br />
espresso come numero compreso tra 0 ed 1 (VEGF (i) = % della<br />
cellularità midollare x % cellule VEGF positive/10 4 ).<br />
Risultati. La valutazione della DMV-HS ha rivelato differenze<br />
statisticamente significative fra i gruppi CC (7,5 ± 3,6),<br />
TE (10,1 ± 4,5), PV (20,7 ± 10,2) e CIMF (25,6 ± 6,3) (p <<br />
0,0001), risultando superiore nei pazienti con PV e CIMF, rispetto<br />
ai CC e TE (p < 0,05). I risultati ottenuti con la metodica<br />
semi-quantitativa sono sovrapponibili (p < 0,001).<br />
Il VEGF (i) ha mostrato livelli di espressione differenti fra i<br />
gruppi CC (0,08 ± 0,009), TE (0,12 ± 0,05), PV (0,28 ± 0,20)<br />
e CIMF (0,29 ± 0,15) (p < 0,001), con valori più elevati nei<br />
pazienti con CIMF e PV rispetto ai CC e TE (p < 0,05).<br />
Una correlazione diretta tra DMV e VEGF (i) è stata identificata<br />
nelle Ph - MPC (r: 0,67; p value < 0,001) e singolarmente<br />
nella PV (r: 0,79; p value < 0,001) e nella CIMF (r: 0,40; p<br />
value = 0,013).<br />
Conclusioni. Nelle Ph - MPC l’incremento della DMV, inteso<br />
come indicatore dell’angiogenesi, correla direttamente con i<br />
livelli di VEGF.
