Zus<strong>am</strong>menfassung<br />
K<strong>in</strong>dliche Frakturen unterscheiden sich erheblich von Frakturen im Erwachsenenalter.<br />
Unfallmechanismen oder Fragmentzahl spielen im Gegensatz zu Frakturen des Erwachsenen<br />
e<strong>in</strong>e untergeordnete Rolle; wesentlich entscheidender s<strong>in</strong>d Frakturlokalisation und<br />
Fragmentmorphologie. Bei noch offenen Wachstumsfugen weichen Frakturmorphologie,<br />
Therapiealgorithmus und Outcome erheblich von denen der Erwachsenen ab. Die bee<strong>in</strong>flussenden<br />
Faktoren der Heilung und Behandlung s<strong>in</strong>d determ<strong>in</strong>iert vom biologischen Alter<br />
und der Wachstumspotenz der Fuge. Der Prozess der Spontankorrektur und des Remodel<strong>in</strong>g<br />
muss prospektiv <strong>in</strong> <strong>die</strong> Behandlungs<strong>über</strong>legungen e<strong>in</strong>fließen. Hierbei s<strong>in</strong>d schon bei Behandlungsbeg<strong>in</strong>n<br />
<strong>die</strong> Grenzen und Möglichkeiten des Remodel<strong>in</strong>g abzuschätzen.<br />
Bei extraartikulären Frakturen kann man <strong>in</strong> gewissen Grenzen Spontankorrekturen erwarten.<br />
So ist <strong>in</strong> vielen Fällen e<strong>in</strong>e anatomisch exakte Reposition nicht erforderlich, andererseits<br />
aber können zu große Fehlstellungen nicht belassen werden. Grundsätzlich gilt, je<br />
jünger das verletze K<strong>in</strong>d ist, umso größere Fehlstellungen können im H<strong>in</strong>blick auf e<strong>in</strong>e<br />
Spontankorrektur akzeptiert werden. Bei allen <strong>in</strong>traartikulären Frakturen darf ke<strong>in</strong>e<br />
Spontankorrektur erwartet werden. Tendenziell geht auch <strong>die</strong> Behandlung von k<strong>in</strong>dlichen<br />
Frakturen mehr zu operativen Stabilisierungen. In jedem E<strong>in</strong>zelfall muss der behandelnde<br />
Arzt sich fragen, ob <strong>in</strong>vasive Maßnahmen erforderlich s<strong>in</strong>d.<br />

Summary<br />
Childhood fractures differ considerably from fractures <strong>in</strong> the adulthood. The mechanism<br />
and the number of fragments are far less essential than <strong>in</strong> fracture of the adult. Of vital<br />
importance however are fracture localization and morphology of the fragment. While the<br />
epiphysis is still open fracture morphology, therapy logarithm and outcome deviate considerably<br />
<strong>in</strong> comparison to adults. The ma<strong>in</strong> <strong>in</strong>fluenc<strong>in</strong>g factor of the cure and treatment<br />
is determent from biological age and growth power of the epiphysis. The process of spontaneous<br />
correction and re- modell<strong>in</strong>g has to be prospectively <strong>in</strong>tegrated <strong>in</strong>to the treatment<br />
thoughts. Therefore possibilities and limits of the re-modell<strong>in</strong>g have to be evaluated already<br />
at the beg<strong>in</strong>n<strong>in</strong>g of treatment.<br />
In extraarticular fractures it can be expected that spontaneous corrections will take place<br />
<strong>in</strong> certa<strong>in</strong> limits. Therefore <strong>in</strong> many cases an exact anatomically reposition is obsolete,<br />
on the other hand too dist<strong>in</strong>ct malpositions can not be tolerated. Basically it applies; the<br />
younger the <strong>in</strong>jured child is, much greater malposition can be tolerated <strong>in</strong> expectation of<br />
spontaneous correction. In all <strong>in</strong>traarticular fractures no spontaneous correction is to be<br />
