Care Planning<br />

This is about identifying client goals and expected<br />

outcomes, which if achieved prevent, reduce or<br />

eliminate the problems identified in the nursing<br />

diagnoses and identify the interventions necessary.<br />

The plan <strong>of</strong> care is then communicated.<br />

It should be underpinned by a framework – which one<br />

are you using?<br />

How many nursing models have you heard <strong>of</strong>? Write<br />

down which ones you remember and how they<br />

worked.<br />

Can you identify similarities between the<br />

two?<br />

The written plan should be<br />

goal-orientated, efficient, effective and<br />

individualised.<br />

Why Care Plan?<br />

Good care planning and care plans can<br />

help nurses to demonstrate that<br />

decisions about care were based on<br />

comprehensive assessment <strong>of</strong> the<br />

patient, systematic nursing diagnosis,<br />

planning <strong>of</strong> care, evidence based<br />

implementation <strong>of</strong> care checking and rechecking<br />

and evaluating.<br />
