A proactive assessment would have the<br />

aim <strong>of</strong> detecting problems at an early<br />

stage in order to initiate interventions<br />

designed to improve health, reduce<br />

disability and functional decline,<br />

improve social participation and improve<br />

the residents quality <strong>of</strong> life.<br />

Needs Planning<br />

A written statement in terms <strong>of</strong> a<br />

residents problem, alteration in health<br />

state or client strength for which caring<br />

provides the primary therapy.<br />

The resident should have ownership <strong>of</strong><br />

the statement – it should be written in<br />

terms that they understand.<br />

Needs Planning<br />

The purpose <strong>of</strong> diagnosing is to<br />

identify:<br />

1. Actual and potential needs in the way<br />

the resident responds to health or<br />

illness.<br />

2. Factors that contribute to or cause<br />

these needs.<br />

3. Strengths that the resident has which<br />

he or she can use to prevent or<br />

resolve these needs.<br />
