Informati<strong>on</strong> Sharing and Use<br />

Recommendati<strong>on</strong> 5-15: The Local Health Integrati<strong>on</strong> Networks must integrate care across<br />

<strong>the</strong> system by sharing informati<strong>on</strong> <strong>on</strong> patients am<strong>on</strong>g health care providers, co-ordinating<br />

decisi<strong>on</strong>s and allocating funds to best reflect regi<strong>on</strong>al needs.<br />

Their mandate should include <strong>the</strong> range <strong>of</strong> services described in Recommendati<strong>on</strong> 5-5:<br />

primary care physicians, acute care hospitals, l<strong>on</strong>g-term care, community care, home care,<br />

public health, etc. The pan-provincial instituti<strong>on</strong>s listed in Recommendati<strong>on</strong> 5-5 should have<br />

<strong>the</strong>ir own authority, which would be accountable directly to MOHLTC, and <strong>the</strong> government<br />

should explore opportunities to c<strong>on</strong>centrate very specialized intensive care into a few centres<br />

to take advantage <strong>of</strong> <strong>the</strong>ir existing talent and infrastructure strengths.<br />

Recommendati<strong>on</strong> 5-16: Use data and informati<strong>on</strong> sharing to better understand and address<br />

<strong>the</strong> fiscal impacts <strong>of</strong> chr<strong>on</strong>ic and complex c<strong>on</strong>diti<strong>on</strong>s and at-risk patients with mental health<br />

and addicti<strong>on</strong> issues (see Recommendati<strong>on</strong>s 5-37 to 5-41 for more <strong>on</strong> managing <strong>the</strong>ir care).<br />

Analysis <strong>of</strong> potential soluti<strong>on</strong>s, including more efficient use <strong>of</strong> <strong>the</strong> full range <strong>of</strong> health services<br />

and o<strong>the</strong>r agencies should be d<strong>on</strong>e in tandem with an assessment <strong>of</strong> <strong>the</strong> potential for greater<br />

involvement <strong>of</strong> <strong>the</strong> private sector in providing advice <strong>on</strong> complex case management.<br />

Integrati<strong>on</strong> and co-operati<strong>on</strong> between <strong>the</strong> health, mental health, addicti<strong>on</strong>, social service,<br />

justice and youth protecti<strong>on</strong> service sectors is crucial.<br />

Recommendati<strong>on</strong> 5-17: Use informati<strong>on</strong> from funding models such as <strong>the</strong> Health-Based<br />

Allocati<strong>on</strong> Model (HBAM) 62 to examine where services may not be provided equally across<br />

health regi<strong>on</strong>s and c<strong>on</strong>duct <strong>on</strong>going evaluati<strong>on</strong>s <strong>of</strong> each Local Health Integrati<strong>on</strong> Network’s<br />

progress in managing high-use populati<strong>on</strong>s. (See Recommendati<strong>on</strong>s 5-50 and 5-73 for more<br />

details <strong>on</strong> HBAM.)<br />

Optimize <strong>the</strong> HBAM data set to identify each LHIN’s high-use populati<strong>on</strong> <strong>on</strong> an annual basis,<br />

including <strong>the</strong>ir specific demographic, socio-ec<strong>on</strong>omic, diagnostic and procedural<br />

characteristics. Use this informati<strong>on</strong> to better understand and learn from <strong>the</strong> differences in<br />

treatment practice in each LHIN and apply best practices across <strong>the</strong> province. Fur<strong>the</strong>r, use<br />

HBAM data to build specific strategies for co-ordinating health care for each high-use clinical<br />

group, for example: end-<strong>of</strong>-life care, avoidable complicati<strong>on</strong>s, and care for those with mental<br />

health and addicti<strong>on</strong>s issues.<br />
