Recommendati<strong>on</strong> 5-15: The Local Health Integrati<strong>on</strong> Networks must integrate care across<br />

<strong>the</strong> system by sharing informati<strong>on</strong> <strong>on</strong> patients am<strong>on</strong>g health care providers, co-ordinating<br />

decisi<strong>on</strong>s and allocating funds to best reflect regi<strong>on</strong>al needs.<br />

Recommendati<strong>on</strong> 5-16: Use data and informati<strong>on</strong> sharing to better understand and address<br />

<strong>the</strong> fiscal impacts <strong>of</strong> chr<strong>on</strong>ic and complex c<strong>on</strong>diti<strong>on</strong>s and at-risk patients with mental health<br />

and addicti<strong>on</strong> issues (see Recommendati<strong>on</strong>s 5-37 to 5-41 for more <strong>on</strong> managing <strong>the</strong>ir care).<br />

Recommendati<strong>on</strong> 5-17: Use informati<strong>on</strong> from funding models such as <strong>the</strong> Health-Based<br />

Allocati<strong>on</strong> Model (HBAM) to examine where services may not be provided equally across<br />

health regi<strong>on</strong>s and c<strong>on</strong>duct <strong>on</strong>going evaluati<strong>on</strong>s <strong>of</strong> each Local Health Integrati<strong>on</strong> Network’s<br />

progress in managing high-use populati<strong>on</strong>s. (See Recommendati<strong>on</strong>s 5-50 and 5-73 for<br />

more details <strong>on</strong> HBAM.)<br />

Recommendati<strong>on</strong> 5-18: Where feasible, services should be shifted to lower-cost caregivers.<br />

Across <strong>the</strong> spectrum <strong>of</strong> caregivers, full scope <strong>of</strong> practice needs to be exercised.<br />

Recommendati<strong>on</strong> 5-19: A broader perspective should be applied to decisi<strong>on</strong>s that are made<br />

<strong>on</strong> <strong>the</strong> scope <strong>of</strong> practice <strong>of</strong> health pr<strong>of</strong>essi<strong>on</strong>als. Government should play a more active role in<br />

working with <strong>the</strong> pr<strong>of</strong>essi<strong>on</strong>al colleges to apply a system-wide approach ra<strong>the</strong>r than dealing<br />

with individual pr<strong>of</strong>essi<strong>on</strong>s in isolati<strong>on</strong>.<br />

Recommendati<strong>on</strong> 5-20: Maximize opportunities to use nurse practiti<strong>on</strong>ers with <strong>the</strong> aim <strong>of</strong><br />

efficiency, while maintaining excellent care.<br />

Recommendati<strong>on</strong> 5-21: Recognize <strong>the</strong> increased demand for nurses in <strong>the</strong> capacity <strong>of</strong><br />

nursing programs at colleges and universities and <strong>the</strong>ir ability to train more nurses.<br />

Recommendati<strong>on</strong> 5-22: Increase <strong>the</strong> use <strong>of</strong> pers<strong>on</strong>al support workers and integrate <strong>the</strong>m into<br />

teams with nurse practiti<strong>on</strong>ers, registered nurses and o<strong>the</strong>r staff members where appropriate<br />

to optimize patient care.<br />

Recommendati<strong>on</strong> 5-23: Local Health Integrati<strong>on</strong> Networks need to use funding as a lever<br />

to encourage hospitals and o<strong>the</strong>r health care providers to use <strong>the</strong> full scope <strong>of</strong> practice <strong>of</strong><br />

<strong>the</strong>ir staff.<br />

Recommendati<strong>on</strong> 5-24: Make changes to <strong>the</strong> Pharmacy Act to enable an expanded scope<br />

<strong>of</strong> pharmacy practice. This would involve developing supporting regulati<strong>on</strong>s to permit<br />

pharmacists to administer routine injecti<strong>on</strong>s and inhalati<strong>on</strong>s, including immunizati<strong>on</strong>.<br />
